GENTLEMEN:

We, Diosdado, Jeffrey, Jovy, Jordan husband/ wife and children, respectively, of
deceased Jovena Empinado Sepe request to be allowed to withdraw the credit balance of account
number 1090915612 in the total amount of Php 200563.81 + 5,909.74 as of March 27, 2023 in
the name of the deceased, subject to six (6%) percent final withholding tax, without any judicial
settlement of the estate of the said decedent, and without waiting for the expiration of the 2-year
period within which any other heirs or creditors may file their claims against the estate of the
decedent, since the said amount will be used to defray part of the funeral expenses incurred at the
wake/ funeral sept of the deceased Jovena Empinado Sepe.

We hereby warrant that the deceased Jovena Empinado Sepe died intestate on February
28,2023 at Balut Tondo Medical Center as evidenced by a certified photocopy of his /her
Certificate of Death attached hereto as Annex "A". Also attached and made integral parts hereof
are the Marriage Contract between and Jovena Empinado Sepe and Diosdado Sepe Sr. and the
Birth Certificates of their children as Annexes “A” to “E”, respectively.

We also warrant that we are the sole compulsory heirs of the deceased and to the best of
our knowledge and belief, the deceased left no debts or obligations whatsoever in favor of any
person, natural or juridical.

In consideration of your allowing us to withdraw the credit balance of the aforesaid


accounts, we hereby jointly and severally agree and undertake to indemnify and/or reimburse the
Bank and save / hold it free and harmless from any and all claims, liabilities and damages, causes
or choses of action, losses, taxes, expenses, fees, penalties, including attorney's fees and the like
of whatever nature and kind which the Bank, may at any time, sustain or incur or be made liable
by reason of the withdrawal of the credit balance of Account Numbers: 1090915612 &
1090917319.
