6.005-2 SAUDI ARAMCO LOSS PREVENTION STATISTICS: DEFINITIONS

1.0 PURPOSE:
1.1 To provide a uniform method of recordkeeping consistent with other established standards
worldwide.

1.2 To measure corporate safety performance and compare it to both the Company's past
performance and to the performance of other petroleum companies.

1.3 To establish a uniform database for the purpose of identifying trends and accident causal factors.

1.4 To provide accurate and timely information on loss statistics to all levels of management.

2.0 RESPONSIBILITIES:
2.1 Loss Prevention Department:

Loss Prevention Department is responsible for receiving, collating, recording and analyzing the
Company's safety performance data and providing periodic reports to management. Data
collected includes employee injuries both on-job and off-job, and Company motor vehicle
accidents.

All departments are responsible for ensuring that the Loss Prevention area superintendent or his
office is advised by telephone or fax of any on-job injury or motor vehicle accident within 1
(one) hour of learning of an incident, as required by G.I. 6.001 entitled, "Notification
Requirements for Incidents (Including Fires)", and G.I. 6.029, "Reporting and Recording of
Motor Vehicle Accidents". Off-job injuries should be reported to Loss Prevention Department
within 24 hours. Departments should initiate other notification procedures and forms per the
appropriate G.Is referenced in Supplement 6.005-1 of this General Instruction. They must also
follow the other provisions of the General Instructions referenced in Supplement 6.005-1,
including investigating causes of incidents and providing and implementing appropriate
recommendations.

2.3 All cases where employees are hospitalized in a private or Government hospital after an injury
or accident shall be reported by the employee's supervisor to the Workmen's Compensation
Division of the Employee Relations Services Department and the area Loss Prevention office,
within 24 hours from the time the injury/accident is brought to his notice.

3.0 GENERAL INFORMATION:


3.1 Abbreviations:

(i) IDI - Industrial Disabling Injury

(ii) RDI - Restricted Duty Injury (on-the-job)

(iii) ODI - Off-The-Job Disabling Injury

(iv) MVA - Motor Vehicle Accident

(v) UCL - Upper Control Limit

(vi) SPI - Safety Performance Index

For detailed definitions of these terms refer to Supplement 6.005-2 or other referenced standards
as indicated in Supplement 6.005-1.

3.2 For corporate management purposes, the measure of the Company's safety performance is
known as the index "CORP-3" which is equal to the IDI frequency.

3.3 On-job hours are those shown on time cards minus the paid off-time hours. Paid off-time hours
include paid time on holidays, scheduled days off (weekends) and the following:

Detailed definitions of the above time categories are given in Saudi Aramco's Time Reporting
Manual.

3.4 EMPLOYEE as used in this G.I. includes all employees in the Employee Information System
(EIS) database whose manhours are charged to a Saudi Aramco organization code through the
Time Reporting System.

4.0 CLASSIFICATION OF PERSONAL INJURIES/ILLNESSES:


4.1 On-The-Job Injuries and Occupational Illnesses: Any injury or illness suffered by an employee
which arises out of or in the course of his employment: i.e., resulting from work activity or the
environment of employment; or any abnormal conditions or disorder, not classified as an injury,
caused by exposure to environmental factors associated with employment (even if the exposure
involves a single incident). Illness includes diseases or rashes which may be caused by
inhalation, absorption, ingestion, or direct contact. The exposure may have taken place over a
period of time or resulted from a single incident. Some examples include noise induced hearing
loss, dust-disease of the lung, respiratory conditions due to toxic agents, poisoning (such as H2S
and other gases), disorders due to physical agents (other than toxic materials) such as heatstroke,
heat-exhaustion, dehydration, or other environmental factors, and illnesses to medical
professionals as a result of exposure to patients.

On-the-job injuries include the following:


4.1.1 IDI: An on-the-job injury or occupational illness shall be classified as an IDI if it results
in a fatality or involves day(s) away from work. The day on which the injury occurred,
or onset of illness, and days off such as weekends and holidays are not included as lost
workdays.
4.1.2 RDI: An on-the-job injury or occupational illness shall be classified as an RDI if it re-
sults in workday(s) with restricted work activity. The day on which the injury occurred
or onset of illness, and days off such as weekends and holidays, are not included as re-
stricted duty days.

NOTE: The interpretation of on-the-job cases shall be in accordance with the


"Recordkeeping Guidelines for Occupational Injuries and Illnesses" (refer to
Supplement 6.005-1, item 6). The interpretation of classifying whether an on-
the-job case is an IDI or RDI shall be in accordance with ANSI Z16.4-1977.
4.1.3 Medical Treatment Injury: Any on-the-job injury which cannot be classified as IDI or

RDI, and for which a Form SA-3208 ("Request for Medical Treatment of Injury or Oc-
cupational Illness") is initiated, shall be recorded as a "medical treatment injury" case by
Loss Prevention Department in accordance with ANSI Z16.1-1967 (R1973).

4.2 Off-The-Job Injury: Any injury suffered by an employee which does not arise out of and in the
course of employment shall be considered an off-the-job injury, except certain cases excluded
by Section 6 of ANSI Z16.3-1973 (e.g., injuries experienced while resisting arrest, attempting to
escape from, or disobeying the orders of an officer of the law or while committing a felony,
legal execution, or injuries resulting directly from enemy action in time of war).

4.2.1 ODI: Any off-the-job injury which results in death or day(s) away from work shall be
recorded as an ODI for Loss Prevention statistical purposes.

NOTE: The interpretation of off-the-job cases shall be in accordance with ANSI


Z16.3-1973.

4.3 The injury classifications listed in this instruction are intended to provide a uniform method for
recording and measuring the Company's injury experience. These classifications may differ
from those used by the Employee Relations Services Department for the purpose of complying
with workmen's compensation laws.

5.0 CHARGING OF INJURIES AND MVAs:


5.1 In general, MVAs shall be charged to the department to which the involved vehicle is assigned
at the time of the accident; that is, the department which is credited with the kilometers driven
on the vehicle, irrespective of the affiliation of the driver. One MVA will be charged for each
vehicle involved. (Also refer to G.I. 6.029, "Reporting and Recording of Motor Vehicle
Accidents".)

5.2 In general, injuries shall be charged to the employee's department as recorded in the Employee
Information System (EIS) database (i.e., the organization code being credited with the
employee's manhours) at the time of the injury or at a time when an occupational illness is
diagnosed by the Medical Organization. One injury will be charged for each employee injured.
Some clarification and exceptions are listed below.

5.2.1 Injuries to employees, e.g. trainees, personnel in the Professional Development Program
(PDPs), Regular Development Program (RDPs), part-time employees (e.g., summer stu-
dents, co-op. students), and non-employee apprentices, are charged to the department
shown in the EIS database at the time of the injury, even if the employees are physically
assigned to different organizations. Restated, the injury follows the SA-8000 "PRO-
FILE".
5.2.2 Injuries to personnel in the College Degree Program (CDPs) are charged to the Career
Development Department, irrespective of the destination department of the involved
CDP.

5.2.3 Injuries to consultants and professional visitors whose on-the-job manhours are not cred-
ited to the proponent department are not charged to any Saudi Aramco department.
5.2.4 Injuries sustained by volunteer firemen while on-call, during fire drills and fire fighting
training, or other related activities, are charged as on-job injuries to the Fire Protection
Department.
5.2.5 Injuries to employees during their paid vacation shall be charged to the employees' de-
partments as ODIs if the employees get "M" days in lieu of the vacation days lost due to
injury. A Form SA-3208 should be initiated by the employee's supervisor and be ap-
proved by the appropriate Medical organization to specify the number of "M" days, and
record details of the injury.

6.0 HANDLING OF DOUBTFUL CASES:


6.1 Any questions or concerns pertaining to compensability of an injury shall be referred to the
Workmen's Compensation Division of the Employee Relations Services Department.

6.2 Any questions or concerns pertaining to whether an injury is serious enough to warrant no duty
or restricted duty as opposed to regular duty, or the duration of absence authorized, shall be
referred to the appropriate Medical organization.

6.2.1 For example, if the attending physician specifies in the Form SA-3208 that the injured
employee is being given a day off (in addition to the remainder of the day of the injury)
then it would be considered a lost workday case (either an IDI or an ODI) irrespective of
the fact that in some cases the injured employee elected to return to work on the days off
given him by the physician. Reclassification of such cases can be done only if the Med-
ical organization submits a supplementary Form SA-3208 as stated in paragraph 6.4.

6.3 Any questions or concerns pertaining to the classification (IDI, RDI or ODI) or charging of an
injury for Loss Prevention statistical purposes shall be referred to the appropriate Loss
Prevention area division head.

6.4 In order to seek an alteration or cancellation of a recorded IDI, RDI or ODI, the proponent
department shall submit the following documents to the appropriate Loss Prevention area office
within two weeks of the date of injury:

i) Legible copies of the completed Form SA-3208, including amendments to any previous
diagnosis and/or classification of the injury/illness.

ii) A legible copy of the completed Form SA-681, "Supervisor's Investigation of a Job
Related Injury or Illness", if applicable.

iii) A written statement from the injured employee describing circumstances and the
sequence of events leading to the injury.

iv) A written statement from the employee's supervisor elaborating reasons for seeking
alteration or cancellation.

v) Copies of statement(s) from witness(es) of the incident leading to injury, if available.

vi) Copies of any correspondence sent to or received from the Medical organization and/or
the Employee Relations Services Department.

vii) Any other information/documentation pertinent to the case.

6.5 The Loss Prevention area division head will conduct a thorough investigation of the case in light
of the documentation submitted, and decide if alteration/cancellation is justified. He shall send
his decision, in writing, to the proponent department, citing relevant section(s) of the applicable
ANSI Standard(s) and/or established precedents. A copy of the decision shall be sent to the
Planning and Technical Services Division of Loss Prevention Department.

6.6 Under exceptional circumstances, if the proponent department manager disagrees with the
handling of doubtful case judgment as specified in paragraph 6.5, he can appeal the case to the
Loss Prevention Department Manager's office. All documentation pertaining to such a case
shall also be sent to the Loss Prevention Department Manager. The Loss Prevention
Department Manager's decision shall be final in recording of injury/MVA data for statistical
purposes.
