Agenda:
a. Adjustment of school hour.
b. Payment for School I.D
c. School activities (Saliyawit, Schools Press-conference)

f. Presentation of Academic Excellence awardees and meritorious awardees
g. Giving of cards cum students’ reiteration of proper school behavior
Proceedings:
I. The Homeroom PTA meeting started at exactly 10 A.M. Parents were asked to
accomplish the attendance sheet for the monitoring of attendees for the said
meeting and card day.
II. Based from the attendance sheet, 73.33% or 11/15 were present and was
suggested to proceed, since quorum was met.
III. The teacher-adviser presented the first school matter- re adjustment of school
hour. Majority or 100% from the attendees agreed for the proposed school hour.
However, concern was raised by the HPTA President, Hon. Leordlyn Asuncion,
that breaktime for the afternoon shall no longer commence (if and when possible)
to aid the teaching hour allotted for the breaktime.
IV. A payment of twenty pesos (20php) shall be collected from the parents to defray
the payment for school ID as their counterpart. No disagreement was noted.
V. Parents and learners were told that there are upcoming school activities re
saliyawit, career guidance and press-conference. In line with these, they agreed
that whenever the school needs their participation, parents will not be hesitant to
allow them join the said activities.
VI. Career Guidance will be held on November 16-17, 2022, student participants were
introduced on the different contests they shall join. In addition, on the second day
of the activity, symposium will happen on the morning and the parade of career
will be in the afternoon. It was also noted that the students shall prepare a career
attire for them on the said day and time of the contest as they will be choosing the
Career Ambassador and Ambassadress.
VII. After discussing the different matters, Academic Excellence awardees were
presented for the first quarter and were awarded certificates and medals. After
which, meritorious awardees were also awarded with certificate of
commendations.
VIII. Temporary cards were released to parents to be able for them to see the progress
of their child for the first quarter of the first semester, this school year.
IX. The meeting was adjourned at 11:00 a.m.

MEANS OF VERIFICATION
Agenda:

c. School Activities (Larong Pinoy Sportsfest 2023, Math Bowl, Division Festival of Talents)

e. Classrooms needs
f. Presentation of Academic excellence awardees and meritorious award awardees
g. Giving of cards cum students, reiteration of proper school behavior and attendance
Proceedings:
I. At exactly 8:00 am, the homeroom PTA meeting began. Parents were asked to fill
out an attendance form for monitoring.
II. 10 out of the 15 attendees listed on the attendance sheet were at the meeting, and
it was proposed to proceed on the meeting because quorum had been reached.
III. The adviser presented the first agenda of the meeting, which was the school
accomplishment like Monthly Holy Mass, Monthly Awarding for the different
categories in school, successful 26th founding anniversary and improved school
gardens.
IV. He also presented the different classroom achievements to the parents.
V. The adviser also discussed the Project playground to the parents. They all agreed
to bring pegs for the fence of the project playground. They also agreed to help
build the fences.
VI. The teacher discussed the classroom needs of the students. Such as floor wax
paint, extension wire and cleaning materials. Thus, the parent agreed to solicit for
the classroom needs.
VII. After the discussion of various school-related issues, the academic excellence
awardees for the 3RD quarter were presented and awarded with certificates.
VIII. Parents were given temporary cards so they could monitor their children's
progress for the 3RD quarter of the school year 2022-2023.
IX. The meeting was adjourned at exactly 9:10 in the morning.

Proceedings:
I. The meeting was started through a prayer as led by Ms. Melody Eslabon.
II. The class advisers gladly congratulated the parents and students for passing the
grade 12 level and also for having a good academic status
III. The first agendum was presented about the PTA fee.He cleared about the issue on
PTA fee and also the aim of collecting PTA fee which will be used to purchase
chairs for incoming school programs.
IV. The parents agreed to pay the PTA fee.
V. In addition, he also explained the payment for the Reading center and with her
explanation, most of the parents were enlightened.
VI. The third agendum, he also clearly explained how students ‘grades are being
computed for the fourth quarter and including the selection of academic awardees
and who will be receiving the meritorious awards/ special awards.
VII. He also stressed about Graduation Ball which was scheduled on July 10, 2023 and
Graduation Day on July 13, 2023.
VIII. Awards to be accorded were also presented to the parents.
