AFFIDAVIT OF ADMISSION OF PATERNITY

I, JOSEPH BENJAMIN E. PAUIG, of legal age, Filipino and a resident of Butuan


City, after having been duly sworn to in accordance with law, do hereby depose and
state.

That I am the biological father of ____________________, who was born out of


wedlock on _________, 2022 at _________________________________, Butuan City
to her mother “Lovely Grace B. Lontoc”.

That I hereby acknowledge the above-mentioned child as my own and he may


bear my surname “Pauig” in accordance with the will of the mother or the law;

That I am currently at work on __________, 2022. Hence, I cannot personally


appear at the City Civil Registrar’s Office, Butuan. Copy of the employment herein
attached and marked as Annex “A” for part of this Affidavit;

That this affidavit was executed in order to attest to the truthfulness of the
foregoing without undue influence from anybody but out from my own free will and
voluntary act and deed.
