i) Photocopy of Performance Rating in the last rating period, if applicable
j) Checklist of requirements and Omnibus Sworn Statement on the Certification on the
Authenticity and Veracity of the Documents submitted and Data Privacy Consent Form
pursuant to RA 10173
k) Means of Verification (MOVs) for Outstanding Accomplishments, if applicable
l) Means of Verification (MOVs) for Application of Education, if applicable
m) Means of Verification (MOVs) for Application of Learning & Development/Training, if
applicable
n) Photocopy of Performance Rating obtained from the relevant work experience, that is,
if the item no. 9 is not relevant to position applied for, if applicable
