hints:
Introduction

Government’s referral system assumes a
pyramidal pattern of referral system
recommended by health Planners that is from
dispensary to consultant hospital.
Main body (7.5
marks)
The following are health facilities from low level
to highest level through which a referral system go
through;
Dispensary: The dispensary services for 6,000
to 10,000 people and supervisors all the village
health points in its ward. This is the first level
where patients are attended to. If the patient
cannot be attended to at this level because of
the complexity of condition is then referred to
the next level which is a health centre.
Health Centre services: A health centre is
expected to centre for 50,000 people which is
approximately the population of one
admiration division. This is the second level
where patients are attended to. If the patient
cannot be attended to at this level because of

the complexity of the conditions then referred
to the next level which is district level.
District hospitals: The district is very
important level in the provision of health
services in the country each district is
supposed to have a district hospital. For those
districts which donate have governments
normally negotiated with religious
organization to designate voluntary hospital
get subventions from the government to
contract terms. Patients are refereed to have
from health Centre and dispensaries present
the district.
Regional hospitals: Every regional is
supposed to have a hospital. Regional
hospital officer similar services like those
agreed at districts levels, however regional
hospital level specialists in various fields and
addition services which are not provided at
district hospitals. Patients from districts are
referred to these hospitals for further
management.

5. Describe five (5) types of irrational prescribing.
Answers:
Introduction:
(1.5marks)
Irrational prescribing means prescribing that
does not conform to good standard of
treatment e.g. extravagant prescribing, over
prescribing, incorrect prescribing, and
multiple prescribing or under prescribing
Main body:
(7.5marks)
Wish u all the best……..
Do not send this document to anyone please……….!!!
i. Extravagant prescribing: this occurs if
the drug is prescribed when
A less expensive drug would provide
comparable efficacy and safety
symptomatic treatment of mild
condition diverts funds from treating
serious illnesses, and
a proprietary product is used where
less expensive generic equivalent is
available
ii. Over prescribing: this occurs if the drug
prescribed when;
The drug is not needed
The dose is too large
The treatment period is too long
The quantity dispensed is too great
for the current course of the
treatment
iii. Incorrect prescribing: occurs if the drug
prescribed when;
The drug is given for an incorrect
diagnosis
The wrong drug is selected for the
indication

iv. Multiple prescribing: occurs if the drug
prescribed when;
Too or more medications are used
where one or two would achieve
virtually the same effect
Several related conditions are treated
where treatment of the primary
condition will improve or cure the
conditions
v. Under prescribing: occurs if the drug
prescribed when;
Dosage is inadequate
Length of treatment is too brief
Needed medications are not
prescribed
Conclusion: (0.5
mark)
Candidate should give summary of above
discussion

A profession is distinguished by the wiliness of
individual practitioners to comply with ethical
and professional standard, which exceed
minimum legal requirements. The role of
pharmaceutical persons is developing and
recognized as the expert on medicines within
the overall health system. To ensure people
maintain good health, through responsible use
of medicines.
To practice as pharmaceutical personnel
means under taking any role, whether
renumerated or not, in which an individual
use his professissional skills and knowledge.
Pharmaceutical practices are not restricted to
the provision of direct clinic care, but also

include working a in a non-clinical
relationship with consumers, patient or care
takers in the discovery, development, and
manufacture of medicines, in maintaining the
supply chain or in procurement. Besides this,
pharmaceutical management or
administration, in education, research,
advisory, regulatory or policy development
roles are considered to be important role to
this cadre.
The code of ethics will therefore apply to
pharmaceutical personnel in all pharmacy
practice setting. At all time, code should
strive to provide the best possible care for
consumers, patients and care takers with due
to regard for the limitations of available
resources and the principles of equity and
justice.
These codes of ethics and profession conduct
for pharmaceutical personnel therefore intend
to reaffirm and state publicity the obligations
that form the basis of the roles and
responsibilities of pharmaceutical personnel.

10. Describe the process of stock taking and
reconciliation of medicines and medical supplies;
Answer:
Introduction: (2
marks)
Stock taking is a process of counting
physically all the items in the store i.e.
medicine and medical supplies.
Main body: (7.5
marks)
Physical count;
The storekeeper makes a physical count of the
stock, and makes an entry on the cards. The
stock check can be done on the routine basic
for all the stock (e.g. once a month)
Wish u all the best……..
Do not send this document to anyone please……….!!!
- It is better of the stock count is done
without looking at any other stock
information and then any difference can be
investigated later.
Theoretical balance;
To calculate the theoretical balance which
should be present, the formula used is:
Theoretical balance = opening balance – stock
issued + stock received
If stock count and theoretical balance agree,
then stock is said to be reconciled.
If stock count and theoretical balance do not
agree the stock is not reconciled.
When the stock is not reconciled: that is when
there is a difference between the stock count
and the theoretical balance, it is vital to
investigate the reasons for the difference.
There may be a simple explanation
(accidental breakage), but it may be that,
there is some theft or pilferage occurring, and
if this is so, then random and un-announced
spot check on the stock balance should be
made.
