In line with that, the feast day of Our Lady of Mediatrix of All Grace will be on the month of September.
Therefore, as the class adviser of Grade 6- Our Lady of Mediatrix of All Grace, I would humbly ask your
permission to allow us celebrate our feast day this coming, September 18, 2023. This is to show our students how
Catholics celebrate the life of our beloved Saints.
I am truly looking forward to celebrating this special day with you. I have no doubt that it will be a day filled with
happiness, delicious food, and the warmth of our shared traditions. May it be a day filled with love, joy, and the
company of cherished pupils, friends, and the faculty and staff. Let us come together to celebrate our shared

As an adviser, I am responsible for ensuring that all necessary arrangements are in place to create a comfortable
and enjoyable environment for our guests. However, due to the size and scale of the event, I am in need of
additional seating, tables, and shelter to accommodate everyone. I kindly request permission to borrow the
following items from you: a) Chairs - 50 b) Tables - 4 c) Tent – 2
I assure you that I will take the utmost care of these items and return them promptly after the event. Any damage
or loss that may occur during the event will be my responsibility, and I will cover the costs of repair or
replacement.

The Maryknoll School of Lupon, Inc., Culture and Arts Club (CAAC) members will conduct a PRACTICE for
the upcoming Culmination of the Buwan ng Wika. This activity aims to provide ample time for the members to
give as spectacular show during the said event.
In accordance with this, we humbly ask permission to conduct the said practice at the MSLu Gymnasium and
allow the students to participate in this event. Regarding the schedule, it is set to be conducted on August 17, 2022
to August 25 at exactly 5: 00 to 6: 00 PM.
