I agree to be suspended if I am caught committing any of the following offenses:
1. Gross misconduct.
2. Assaulting a teacher or any other school authority or his agents or students.
3. Drinking intoxicants and liquor.
4. Gambling of any sort.
5. Smoking/using of vape inside the school premise.
6. Wearing of earrings (by male students).
7. Sporting long hair (by male students).
8. Hair dyed with loud color and make-up.
9. Not wearing the prescribed school uniform.
10. Not wearing the school I.D.
11. Cheating during examinations.
12. Extortion or asking money from others.
I further agree and accept to be transferred to another school for any of the following offenses:
1. Using possessing and selling of prohibited drugs.
2. Carrying and concealing deadly weapons.
3. Vandalism
a. Writing on the walls, posts, benches, doors blackboards, chairs and tables.
b. Destroying school property, such as chairs, tables, electric fans, switch of lights and fans,
blackboards and other facilities.

5. Membership in fraternity/sorority. This is punishable by expulsion in accordance with DECS Order NO. 20, s 19
6. Bullying (in any form) / Violation of R.A. 10627, otherwise known as "Anti-Bullying Act of 2013".

SCHOOL RULES AND REGULATIONS
(To be strictly observed by all students)
1. All students are expected to attend their classes on time daily.
2. All students should comply with the requirements of the different subject areas.
3. All students are expected to respect school authorities, teachers, personnel and other school officials.
4. Students should wear the prescribed uniform in school
5. The school I.D. should be worn by the students in the school at all times.
6. Boy's haircut should be short and neat.
7. Students should not come to school with hair-dyed with loud colors and make-up.
8. Male students are not allowed to wear earrings.
9. Smoking and gambling of any form on campus are strictly prohibited.
10. Drinking intoxicants and iquors in school is prohibited.
11. Students are not allowed to go out of the campus during recess time.
12. Carrying and concealing of deadly weapons are strictly prohibited.
13. Using, possessing, and selling of prohibited drugs is strictly prohibited.
14. Avoid public display of affection
15. Students should take good care of the school properties in the classroom and in the campus.
16. All students must wear their school uniform properly.
17. Membership in fraternities/sororities is prohibited in school (per DECS Order No. 20, s 1991).
18. Bullying in any form (per DepEd Order No. 55, s 2013).
DUTIES AND RESPONSIBILITIES OFSTUDENTS
(Section 9, DepEd Order No. 40, s 2012)
Students and learners shall have the following duties and responsibilities:
A. Comply with the school's regulations, as long as they are in harmony with their best interests.
Students and learners shall refrain from:
1. Engaging in discrimination, or leading a group of students to discriminate another, with reference to one's
physical appearance, weaknesses and status of any sort;
2. Doing any act that is inappropriate or sexually provocative;
3. Participating in behavior of other students that is illegal, unsafe or abusive.
4. Marking or damaging school property, including books, in any way;
5. Engaging in fights or any aggressive behavior;
6. Introducing into the school premises or otherwise possessing prohibited articles, such as deadly weapons,
drugs, alcohol, toxic and noxious substance, cigarettes and pornographic materials;
7. Performing other similar acts that cause damage or injury to another.
B. 1. Conduct themselves in accordance with their levels of development, maturity, and demonstrated
capabilities, with a proper regard for the rights and welfare of other persons.
2. Respect another person's rights regardless of opinion, status, gender, ethnicity, religion, as well as
everyone's
moral and physical integrity; and
3. Observe the Code of Conduct for students and learners.

son/daughter in school.
