Please quote your lowest price inclusive of VAT on the item(s) listed below, subject to the Terms and Conditions of this RFQ, and submit or send
your quotation duly signed by your representative, not later than August 10, 2023 at 3 o'clock in the afternoon. Upon receipt of at least ( 1) quotation
after the submission deadline, the BAC shall open the quotation and proceed with the evaluation.

GENERAL CONDITIONS:
1 All entries must be hand written.
2 Approved budget of the contract is P/ 15,000.00 (Lump Sum).
3 Delivery period within TEN (10) calendar days from the date of receipt of Notice to Proceed.
` 4 Warranty shall be for a period of six (6) months for supplies & materials,
one (1) year for equipment, from the date of acceptance by the Procuring Entity.

5 All transactions are subject to withholding of creditable Value Added Tax and/or Expanded Value Added Tax per
revenue regulation(s) of the Bureau of Internal Revenue.
6 Price validity shall be for a period of thirty (30) calendar days.
7 Submission on the following:
a. Philgeps Registration Certificate D. TIN-BIR Registration
b. Income/Business Tax Return
c. Mayor's Permit
