LABELING

- For products with a neck, country of origin must be located on the inside
center of the neck. For bottoms, country of origin must be located on inside

conspicuous and readily accessible location.
- The country of origin label should not be covered or obscured by any other
label.

Packaging (When packaging is present and country of origin is not visible


through the package):
- Country of origin must be written legibly in English and securely attached.
- If any country, other than the country in which the article was
manufactured or produced, is mentioned on the package, the country of
origin must be stated in comparable size and close proximity.
- Must be visible at point of purchase.
- Country of origin on package must be consistent with country of origin on
the product label.

or separate label.
The submitted Sterilization permit will be verified against Permit # on label.
The permit number will be either a PA number or a UT number. If it is a PA
number they still need to register with UT. (PER. NO. UT XXXXX (two
letter country abbreviation) or PA XXXX (two letter country abbreviation).
