Memo - NEW COMPANY ID

Effective December 1, 2023, all employees of Greenwoods Manpower Services must wear their company identification card at all times while on duty to promote safety and security. Employees needing a new or replacement ID must fill out a requisition form along with any necessary documentation, such as an explanation letter for a damaged ID, marriage certificate for changed personal information, or affidavit of loss and payment for a lost ID.

To promote safety and security by ensuring all employees are identified as employee of
Greenwoods Manpower Services, effective on December 1, 2023, all employees must
wear the NEW Identification Card (ID) and shall be considered as part of the prescribed
uniform. All employees are supplied with Company ID that must be worn and visible at all
times when on duty.

Any existing employee that would like to request a new ID or needs a replacement card
shall fill-out an ID Requisition Form and comply with prescribed requirements if necessary.

a. If damaged ID, a signed explanation letter attached with ID Requisition Form.


b. If request for change of personal information, filled-out ID Requisition Form
attached with supporting document such as Marriage Certificate, proof of
residencies, verified specimen signature, change of emergency contact details,
and/or other related document.
c. If lost ID, an Affidavit of Loss and P150 to pay.
d. If changes in employment details, the HR Personnel shall process the update and
new ID of the employee.
