COMES NOW, the Petitioner JOSE M. MONDRAGON, Filipino, of legal


age, married, with residence and postal address at Barangay Bagong Bayan,
Puerto Princesa City, before this Honorable Office avers,

Petitioner is the registered owner of a certain parcel land located at


Bgy. Bancao-Bancao, Puerto Princesa City, covered by TRANSFER
CERTIFICATE OF TITLE NO. 12854, LOT 1-B-2-F, PSD-04-033449 with an
area of One Thousand (1,000) Square Meters;

Petitioner caused the subdivision of the same into five (5) lots under
approved subdivision plan number PSD-4B-014811 and describe as follows;

1. LOT 1-B-2-F-1 200 square meters


2. LOT 1-B-2-F-2 200 square meters
3. LOT 1-B-2-F-3 200 square meters
4. LOT 1-B-2-F-4 200 square meters
5. LOT 1-B-2-F-5 200 square meters

WHEREFORE, Petitioner respectfully request for the splitting of TCT No.


12854 particularly for LOT 1-B-2-F-2 as per approved technical descriptions
hereto attached.
