both of legal ages, Filipino and residents of Tagum City, Davao del Norte, after
having been jointly sworn to in accordance with law do hereby depose and state,
that:
1. We personally knew the deceased, NESTOR E. MANATAD who died on
JUNE 26,2022, in Purok Sampaguita 2,Visayan Village,Tagum City Davao
del Norte;
2. We are neither related to the above-named deceased by consanguinity
and/or affinity.;
3. NESTOR E. MANATAD was a bona fide member of the Social Security
System;
4. Unfortunately, in the Senior citizens I.d. of the deceased, the name of
NESTOR E. MANATAD was erroneously entered as CARLITO E.
MANATAD.

The true and correct name of the deceased was NESTOR E. MANATAD,
not CARLITO E. MANATAD , as reflected in his own Certificate of
Baptism and Certificate of Death, and other public and private documents;

5. NESTOR E. MANATAD and CARLITO E. MANATAD refer to one and

the same person.

6. We are executing this affidavit to attest to the foregoing narration of facts

and for whatever legal purposes and intents this may serve.

Poblacion District, Davao City;
8. We are neither related to SABINO GIBERTAS SILVOSA by consanguinity
and/or affinity.;
9. SABINO GIBERTAS SILVOSA was a bona fide member of the Social
Security System;
10. Unfortunately, in the records of the SSS, the middle name of SABINO
GIBERTAS SILVOSA was erroneously entered as HIBERTAS;
11. The true and correct middle name of the deceased was GIBERTAS, not
HIBERTAS, as reflected in his own Certificate of Live Birth and Certificate
of Death, and other public and private documents;
12. SABINO GIBERTAS SILVOSA and SABINO HIBERTAS SILVOSA refer
to one and the same person.;
13. We are executing this affidavit to attest to the foregoing narration of facts

16. BRAULIO SANCHEZ ESLAIS was married to ELGIE CELESTIAL
ESLAIS.;
17. However, it was discovered lately that the name of BRAULIO SANCHEZ
ESLAIS was erroneously entered as CHIERAY S. ESLAIS, in the
Certification of Marriage issued by the Office of the Civil Registry of Bislig,
Surigao del Sur.;
18. The true and correct NAME of CHIERAY S. ESLAIS, as reflected in his
Driver’s License, UMID, and other public and private documents, is
BRAULIO SANCHEZ ESLAIS.;
19. CHIERAY S. ESLAIS and BRAULIO SANCHEZ ESLAIS refer to one and
the same person.;
20. We are executing this affidavit to attest to the foregoing narration of facts
and for whatever legal purposes and intents this may serve.
