A 17-year-old female presented with intermittent pain in her upper front region. She had a history of mild pain and occasional swelling after a root canal treatment on tooth number 22, which had extruded gutta percha approximately 4 mm and a large periapical lesion extending to teeth numbers 21 and 23. The treatment procedure involved local anesthesia, accessing and cleaning the root canal, removing the extruded gutta percha, enlarging the apical diameter, irrigation, temporary filling, and follow up drainage and filling sessions over 3 years, resulting in resolution of the periapical lesion.
