Health Information Management (HIM®) Career
Opportunities with the Electronic Health Record (EHR)

Introduction
Electronic Health Records (EHR) represent a significant change in health care delivery,
health care facilities operations, documentation practices, and the Health Information
Management (HIM) profession. The EHR is not a suite of computer hardware and
software; it represents “…a carefully constructed set of components that are highly
integrated and require a significant investment of time, process change, technology,
and human factor reengineering”1. Health information is the cornerstone of the EHR,
and the HIM professional is an essential component of EHR systems. This evolution
will create many new roles with new titles as well as modified existing roles and will
continue. Many opportunities await HIM professionals positioned to capitalize on
their education and expertise to secure these new positions.

HIM professionals must continuously evaluate and upgrade their skills and expertise
to keep pace and be successful in the virtual HIM practice.2 HIM careers today require
leadership and management skills, project management skills, data and information
analysis, and knowledge of information technology. With each advancement in
technology, or the advent of new clinical areas of expertise, there is a concomitant
need for new or enhanced skills and knowledge in managing the related information,
whether for clinical care or for research, education, administrative purposes, or
funding.

Health Informatics and Health Information Management Human Resources Report – a


joint report through CHIMA, COACH, ICTC (Information Communications and
Technology Council), CIHI, Canada Health Infoway, Health Canada and the
Information Technology Association of Canada (ITAC Health) - outlined the human
resource requirements and future roles for Health Informatics HI and HIM
professionals. The sector study confirmed a growing demand for new skills and a
severe shortage of HI and HIM professionals (O’Grady 2009). Study findings
suggested that these skills deficits will delay the implementation of the EHR in Canada

without developing the human resources necessary to support its implementation
and maintenance.3

CHIMA validation activities included a comprehensive review of Alberta Health


Services Workforce Transformation activities and competency profiles created
through ICTC. The leadership of Canadian HIM professionals and key external
stakeholders confirmed that these new roles are the future for HIM professionals. It
is evident that the ‘traditional’ HIM roles are expanding in scope and complexity, and
new roles in eHealth Standards, eHealth Privacy, Identification Management and
Decision Support are just the beginning of an expanded scope of practice for the HIM
professional.

The sample descriptions of new HIM roles contained in this PPB are based on several
initiatives including:
• an informal survey of AHIMA’s HIM Practice Transformation e-HIMTM Work
Group;
• CHIMA’s Workforce Transformation project (see Transforming Health
Information Management The Evolution of the HIM Professional, CHIMA 2013);
and
• CHIMA’s PPB on Health Information Management and Health Information
Technology.

The list of sample descriptions is not comprehensive but does provide insight into the
scope of positions in which HIM professionals are employed and will be employed in
the future. The opportunities for HIMs to forge new career paths are endless. The
challenges are to develop new skill sets, to apply existing skills in new ways, and to
have the confidence to welcome changes.

All these roles build on the acknowledged skills of the core HIM curriculum. This
curriculum is based on the Learning Outcomes for Health Information Management
(LOHIM) competencies for accredited HIM programs and graduates who are ready to
take entry level positions within HIM. Many of the new roles will require continuing
education to learn and apply new skills and knowledge. Appendix A outlines the
general requisite entry level knowledge and skills necessary for a career in HIM.

The diagram below offers a visual representation of how skills and education overlap
to make HIM professionals uniquely qualified to assume careers in the world of e-
HIM. HIM professionals must challenge themselves to continuously evaluate and
upgrade their skills and expertise to keep pace and be successful in the world of
virtual HIM practice. HIM careers today employ expertise in three different areas:
health system knowledge (including health services and the biomedical sciences),
leadership and management skills, and knowledge of information technology
systems.

HIM

These future EHR roles have been defined based on the requirements and functions
necessary to accomplish comprehensive and meaningful information management
within any type of health care organization and have been placed in eleven broad
functional or service categories. These are by no means complete; the electronic
landscape is constantly evolving and dynamic, and new roles in areas such as data
collection, information integrity, identity management, data analysis, clinical
documentation improvement, social media, and terminology specialist are a few roles
that will emerge as new technologies are more completely deployed and integrated
into health services delivery.

Data Collection
The data collection service performs all tasks associated with the collection of health
data. This includes data coded directly from clinical documentation or auto-coded
using systems for that purpose, voice and speech recognition systems and scanning
for creating a digital image of text-based documents. This service performs audits of
the service functions to ensure that best practice and current standards are
implemented.

Data Profiler/Mapper
• Assess quality of source data before it is used in data warehousing
• Analyze content, structure and relationships between, and among, data to
uncover patterns and rules, inconsistencies, anomalies and redundancies
• Generate mappings and transformations that perform data correction based
on data rules and selected cleansing strategies
• Identify, analyze and report/resolve data issues- related to uniqueness,
integrity, accuracy, consistency and completeness

Clinical Analyst
• Format, design, and build relevant content into appropriate workflows
• Lead discussions on information and functional design within multidisciplinary
teams
• Provide key knowledge on implementation of standardized, streamlined
content
• Design clinical content for software applications across all care processes
(including documentation, ordering, alerting and notification actions, reporting
needs)
• Develop future state processes and knowledge structures
• Test and troubleshoot problems within applications
• Develop detailed content design documents
• Facilitate validation sessions with clinical researchers
• Develop and publish supporting documentation

Concurrent Classifications Specialist


• Perform concurrent coding on health records in all service delivery
environment
• Convert documentation into diagnosis codes and intervention codes using
appropriate coding systems, for example, ICD-10-CA and CCI codes

• Identify documentation improvement opportunities in the concurrent medical
record

Clinical Documentation Specialist


• Work closely with Care Management staff and clinicians to design accurate and
efficient documentation processes
• Prompt clinicians regarding missing, unclear, or conflicting record
documentation as well as assist in obtaining additional documentation as
needed

Clinical Research Associate


• Monitor activities at clinical study sites to ensure adherence to good clinical
practices, standard operating procedures, and study protocols
• Review regulatory documents as required and prepare site visit reports
• Ensure all required ethics/research board approvals are received
• Participate in the study development and start-up process, including review of
protocols and designing or reviewing study follow-up
• Prepare informed consent forms
• Develop study documents
• Organize and present at investigator meetings
• Work with management personnel on monitoring strategy
• Develop project-specific clinical research associate training

Records and Information Analyst


• Provide reference services to all departments and levels of personnel, as well
as process incoming information
• Sort, classify, and verify coded material for integration into systems
• Create and maintain logs, computerized indices and databases to provide
accurate status and retrieval information

Note: Other new roles under the Data Collection category could include Medical Text
Editor, Clinical Data Specialist, Imaging Clerk, Code Mapping Specialist, Data Mapping
Specialist, Clinical Data Analyst and Central Intake Coordinator.

Information Integrity
This service evaluates the integrity of health information holdings, identifies risks and
implements risk mitigation strategies to ensure that data/information has not been
altered since the last authorized access. This service responds to inquiries into
information integrity and completes an auditing role for both internal and external
requests. This service is closely linked with Information Access, Disclosure and
Privacy.

Data Integrity Specialist


• Identify data requirements and perform structured/random quality audits
across multiple software applications to validate accuracy of data sets,
interfaces and work queues
• Verify that medical records are disclosed appropriately, and are destroyed as
per destruction policies, that transcribed and imaged documents meet quality
standards, and data is collected according to data definitions and guidelines
• Manage revisions to electronic medical records, and correct critical data
elements identified as incorrectly documented and/or scanned
• Track various data errors, complete an analysis, and recommend corrections
• Design statistical trending and performance improvement reports for
management
• Serve as a liaison between management and HIM staff for process
improvement activities

Risk Management Specialist


• Identify and assess potential risks that threaten the information of the
organization
• Identify and assess potential organization and staff risks
• Analyze information related to incidents investigations to improve patient
safety
• Lead the implementation of information and patient safety practices

Senior Document Coordinator for Adverse Events


• Generate reports and manage EHRs associated with compromise of patient
safety due to adverse events such as privacy and/or security breaches, and
natural disasters
• Participate in continuous improvement of departmental processes and
procedures relating to adverse events

• Track timelines for completion of adverse event processing and enter data into
the appropriate adverse events database

Note: Other new roles under the Information Integrity category could include Health
Information Safety Officer and Forensic Investigation Officer.

Identity Management
These roles involve managing systems (e.g. Client Registry) responsible for the unique
identification of clients, providers, organizations and services.

Identity Management Coordinator


• Provide knowledge to technology and administrative teams regarding best
approaches to perform identity matches
• Implement and manage an identity management framework for existing and
new computing services, programs and applications
• Coordinate the use of a common identity and access management platform
across multiple teams with differing technical abilities
• Provide skilled technical development of an identity and access management
architecture

Data Quality Analyst


• Collect, prepare and present information related to client, provider and
organization identification to support quality improvement, and the reporting
and funding needs of the organization
• Use health informatics tools (e.g. dashboards, root cause analysis, data mining,
performance measures) to gather, analyze and report identification ,
operational, and administrative data
• Track and report on key performance and quality improvement measures e.g.
duplicates, overlays
• Collaborate with operations and administration staff to monitor and evaluate
quality improvement initiatives e.g. patient matching accuracy
• Perform data validation and reconciliation to ensure that data/records have
been correctly matched across systems to the correct individual.

Note: Data Integrity Specialist is another role under the Identity Management
category.

Access, Disclosure and Retention
This service protects and promotes data and information privacy for consumers,
providers, users, and the organization. Roles include managing the access, disclosure,
retention, archiving and destruction of Personal Health Information from
organizational systems.

Information Privacy Coordinator


• Monitor organizational information privacy compliance issues
• Work in collaboration with managers of technical information security to
assess risks to health information security and privacy
• Draft information privacy policies and procedures, and champion their
approval and adoption by operational departments
• Develop training and monitoring programs to ensure employee awareness
while complying with health information privacy policies and procedures
• Conduct staff training and evaluations of privacy compliance

Information Security Manager


• Develop and implement the organization’s information security policies and
procedures
• Provide security training to all staff, contractors and third parties
• Report problems to appropriate department managers/administrators
• Ensure appropriate information access levels and security clearances are
maintained
• Prepare the organization’s disaster recovery and business continuity plans for
information systems
• Monitor changes in legislation and accreditation standards that affect
information security
• Initiate, facilitate and promote activities to foster information security
awareness within the organization

Note: Other new roles under the Access, Disclosure and Retention could include
Release of Information Analyst, and Access and Disclosure Specialist.

Information Management Governance


Using best practices and information management principles, this service develops,
maintains and monitors the accountability framework for information. It includes

policies, processes, standards and metrics to ensure unified data and information, as
well as efficient, effective and secure information assets.

Health Information Services Specialist


• Perform regulatory and accreditation review for the health information
services department
• Summarize findings and report results
• Provide technical and administrative assistance for other health information
services functions
• Audits data systems to ensure data rules are being followed
• Develop policies (data linkage, date use and reuse, disclosure and redisclosure,
corrections) to facilitate information integrity and to ensure availability needs
are met
• Develop of online forms; online charting to facilitate data that is ‘fit for use’
• Help to establish work flows that work in the virtual environment
• Help to identify and address appropriate data uses
• Prioritize data needs and access of users

Note: Other new roles under the Information Management Governance category
could include Information Management Liaison, Standards Coordinator, Policy Analyst
and Auditor.

Content Compliance

This service encompasses the management of clinical content to ensure that the
content meets information management best practices including quality evaluation
and improvement audits. This is the bridge between the provider’s clinical and
content needs and the organization’s information requirements.

Compliance Officer
• Audit research investigator activities on site
• Review data collection screens to ensure required data is identified, defined
and standardized
• Verify entries by validating accurate source documentation
• Review data security and protection of Protected Health Information and
Personally Identifiable Information
• Review informed consent forms for completeness and observe consenting
procedures

Note: Another new role under the Content Compliance category could include
Content Management Expert.

Information/Knowledge Asset Management

This service is focused upon developing, maintaining, auditing, and evaluating clinical
information and knowledge assets.

Clinical Vocabulary Manager


• Create, implement, and maintain terminologies, validation files and maps for a
variety of uses in the EHR
• Perform ongoing review of the auto and manual encoders for terminology and
classification systems, and implement recommendations for optimizing the
encoding process
• Assist in the analysis of the enterprise’s classification and grouping system
assignment trends and use data from classification and grouping systems to
assist in decision making
• Proactively monitor developments in the field of clinical terminologies and
medical vocabularies to ensure understanding by users
• Recommend the most appropriate classification or terminology systems to
meet all required information reporting needs
• Demonstrate expertise in clinical terminology, medical vocabulary, and
classification systems
• Adept skills in data mining, or engineering clinical ontologies

Health Systems Specialist


• Utilize various medical data repositories and automated data systems for
sources of information on patient demographics, nature of care provided, case
mixes, relative weight products and effectiveness of ancillary services
• Develop projects improving business practices; thus improving data reliability,
validity and timeliness
• Conduct studies of regional health care delivery systems and operations
• Develop methodologies to track and analyze performance relating to quality of
service, access to inpatient and outpatient care, health care delivery costs and
efficiency of ancillary services

Business Intelligence Specialist


• Analyze organizational data to support performance improvement, patient
safety and service delivery
• Design integrated databases
• Design and implement reporting and analysis solutions

• Collaborate with administrative and clinical staff to create and use data-driven
applications
• Extract data from warehouses using analytic methods
• Work directly with business partners in defining reporting requirements
• Deliver reporting solutions for special projects and ad-hoc

Consumer Support
This service promotes excellent public relations by fostering and maintaining positive
communication with consumers in the areas of Personal Health Record (PHR) and
Registration Systems (registration, appointment booking, and self-registration).

Health Information Broker


• Act as an intermediary between a client (policymaker, patient, administrator,
practitioner) and a desired product or service related to their health
information
• Identify client information needs and recommend products and services to
meet those needs
• Protect client confidentiality and maintain highest level of ethical conduct in
compliance with health organization’s policies and procedures
• Negotiate with client and create contract(s) for selected services
• Integrate genomic data into the HR or PHR
• Something to do with our role with health literacy

Note: Other new roles under the Consumer Support category could include Registry
Agent, Portal Agent, Customer Service Agent, Social Media Speciality, Health System
Navigation Officer, Registration Application Coordinator and Information
Reconciliation Officer.

Application Design and Implementation
Using best practices, this service aids in the delivery of high quality health informatics
applications. The roles cover the entire life cycle of applications including design,
integration with existing systems, implementation and application support.

Clinical Applications Specialist


• Implement and support multi-service software packages
• Manage the customization of site parameters
• Address integration issues with other software packages
• Review site parameters and local tables with each service for accuracy and
completeness
• Coordinate implementation of new software products that cover these
functions
• Analyze and evaluate processes related to information flow
• Lead (or participate in) change management activities related to new
products/services
• Provide training to all services on current software applications and new
features
• Ensure customized role-based training is scheduled for new users
• Coordinate efforts to correct deficiencies and errors that occur in the EHR

Enterprise Applications Specialist


• Demonstrate knowledge of existing and emerging requirements related to
privacy and confidentiality of health information
• Demonstrate knowledge of existing EHR policies and procedures associated
with the collection and distribution of clinical data via the repository
• Facilitate the identification, creation, implementation, and maintenance of
organizational policies and procedures related to the EHR
• Serve as a clinical information liaison to facilitate communication between
clinical caregivers and technical implementation teams
• Serve as an EHR consultant to the organization
• Coordinate and assemble appropriate personnel for task force assignments
related to EHR projects and issues
• Coordinate activities related to the integrity of the master patient index
• Provide developmental guidance and coordination to ensure clinical data
generated and stored in disparate clinical systems is made available in the EHR
when appropriate

Optical Imaging Coordinator


• Assist in the implementation of new optical imaging systems
• Lead change management activities related to new products/services
• Train users and provide follow-up training when required

• Function as a liaison between health informatics and information services
departments

Healthcare Interoperability Specialist


• Design and develop solutions for complex clinical data integration problems
• Design end to end integration solutions to move data between health care
systems using current standards
• Implement best practices for interface documentation, architectural artifacts
for the applications, process and troubleshooting
• Deep technical knowledge of traditional (HL7) and the next generation
integration technologies in use is required
• Demonstrate expertise in scripting, programming and IT networking
• Knowledge and experience in implementing standards based solutions using
exchange protocols

Project and Program Management
This service is responsible for the management and oversight of HIM projects and
programs. These roles provide leadership to a wide variety of teams and acts as a
liaison between employees, project teams and senior management.

Clinical Project Manager, Senior Project Manager


• Manage the scope of work, objectives, and other project management
activities of assigned projects
• Ensure project alignment with organizational strategic plan/mission
• Finalize project budget and scope of work
• Act as a primary project contact with sponsor to ensure appropriate
communication channels are maintained and reporting schedules are adhered
to
• Manage project budget to meet financial and organization goals (realization
targets); identify and develop change orders
• Develop project plans, schedules, and other scope definition documents for
assigned projects
• Develop communication plans to advise stakeholders on system changes
• Maintain project management information and tracking systems
• Coordinate and evaluate the probability and impact of risks using a structured
risk analysis methodology

This service represents the main interface between the client and those responsible
for designing and implementing health informatics systems. With a focus on the
client’s requirements, these roles ensure that business needs are satisfied and that
users are supported during the lifecycle of the system.

Systems Analyst
• Determine client’s business and technical requirements
• Identify appropriate stakeholders
• Consult with client on alternatives to target business needs and opportunities
• Evaluate, recommend, and redesign processes
• Collect and define requirements
• Translate functional design documents
• Participate in the technical design, test planning, and user documentation
processes
• Participate in analysis of client-identified issues or problems, which may
require changes to procedures, standards, or systems
• Analyze business and user needs, document requirements, and revise existing
systems logic difficulties as necessary
