(5) Transfer Certificate and Conduct Certificate issued by the Head of the Institution last studied.

(6) Recently renewed certificates in support of claim for admission under Special Quota (PWD, ESM, FF, MSP, EWS and
Regional Reservation) if applicable.

(7) Recent passport size photograph.

Important:

1. The allotment of seat is purely provisional. Allotment to a candidate who does not possess the required qualification will be CANCELLED,
if it has been made either based on the FALSE data/certificate or by oversight.

2. If the candidate does not join the course offered on or before the date indicated above he/she shall forfeit the seat allotted and he/she
cannot claim the same seat/allotment subsequently.

3. Refer Fee Structure for UG Medical (2023-24) link available in the Important Note Section of the CENTAC Website. Deposit the fee to the
CENTAC account (Refer Press Release) before the last date of joining.
