Dear Ms Goody,
Re: Ms Betty Weston, DOB: 12 February 1964

I am writing to refer Ms Betty Weston, who has been experiencing pain in her right hand. She
now requires your further management.

Ms Weston initially presented to the outpatient clinic on 10 June with numbness and tingling
sensation as well as pain in her right thumb, index and middle finger. She reported that the
pain had been lasting for 3 weeks, exacerbating by gripping objects and causing sleep
disturbances. In addition, she denied any trauma. On examination, Phalen’s manoeuvre and
Tinel’s sign were both positive, which confirmed the diagnosis of carpal tunnel syndrome.

Today, Ms Weston re-visited the clinic. At that time, she was advised to continue her OTC
pain reliever, paracetamol as required. Additionally, she was referred to physical therapy and
further neurological tests. Subsequent night-time wrist splinting as conservative management
for more than 3 weeks was recommended.

In terms of her background, she is a supermarket manager, which requires being on her feet
all day. Please note, her BMI is 32 kg/m2, which aggravates her symptoms.

It would be appreciated if you could provide Ms Weston with a custom-made wrist splint in
the neutral position and ergonomics assessment.
