The undersigned agrarian reform beneficiaries (ARBs) hereby request for the

1. That the undersigned Agrarian Reform Beneficiaries have voluntary given their
consent for individual titling to avoid conflicts and for easy transactions in the
future;
2. That the undersigned Agrarian Reform Beneficiaries will undertake the payment
of the real property taxes due in connection with their respective lot allocations;
and
3. That the undersigned Agrarian Reform Beneficiaries will undertake to carry all
other responsibilities attached in this award.
Attached are the pertinent documents to support our request/petition for parcelization:

Program Officers, the request will be addressed to the Provincial Agrarian Reform Program
Officer II (PARPO II).

ARB/Cooperative/Association/Other Forms of Farmer’s Collective
Organization, if ODC was lost by either of the same group

owners, consenting to the parcelization/subdivision of the subject CCLOA to pay
for the cost and other fees incurred related to the subdivision/parcelization
survey;
___/ Registry of Deeds (ROD) – certified true copy of CCLOA subject for
subdivision/parcelization.
