TO WHOM IT MAY CONCERN:

This is to certify that Michael Capalungan Galanza, 28 years old, male,


resident of Brgy. 20 Pulangi, Bacarra, Ilocos Norte, was carefully examined and
treated at Bacarra Medical Certificate on February 8, 2024 with the following diagnosis:
 No palpable pulses.
 No heart sounds on auscultation.
 No observed respiratory effect.
 No breath sounds on auscultation
 Pupils dilated and not reactive to light.

And would not need medical treatment for over a period of days because he is
already dead.

This medical certificate issued upon the request of Bacarra Municipal Police
Station for the support of the undergoing investigation of Poisoning Incident reported in
this station.
