Dear Ms. Linda Gold,

I am writing a referral letter of Mrs. Kylie Weiss who was diagnosed with Myocardial Infarction.
In behalf of Mrs. Kylie Weiss, I am requesting to visit her at home to assess her eligibility for
availing sickness allowance and other benefits from the Australian government Department of
Human Services.

The patient was admitted on 07, July 2017 with a complaint of discomfort in upper extremities
and in her jaw. During admission, she underwent emergency angioplasty. Post operatively, she
appeared confused about her diagnosis and she was guided from the event occurred and provided
health teaching about her diagnosis. Currently, she was prescribed with series of medications to
be taken at home.

As per patient’s social background, she is married with a daughter. Her husband is 67 years of
age, retired and is receiving pension. She works mixed shifts as a taxi driver. As per patient’s
medical background, she has a mild osteoarthritis and dyslipidemia.

I would like to request to visit Mrs. Kylie Weiss for her concern about being unable to manage
home on her husband’s pension and due to unable to work for six weeks. Also, look for her
eligibility for sickness allowance and benefits from the Australian government Department of
Human Services. Her home address is at 8758, Pulteney Street, Adelaide, SA, 5000.
