AFFIDAVIT OF WAIVER
I, CRISTINA H. LAUREANO, of legal age, single, Filipino and a resident of
Cauplasan, Sta. Maria, Pangasinan, Philippines, under oath, depose and state, that:
1. I am an Overseas Filipino Worker (OFW) presently employed at Kuwait. My
employment therein is duly registered with the Office of the Philippine Overseas
Employment Administration (POEA);
2. I came home to the Philippines for a short vacation with the intention to return to
Kuwait to continue my work therein. In returning to said place, I applied for an
Overseas Employment Contract (OEC) with the office of the POEA-CAR. The
latter required me to produce my employment contract which is a requirement
prior to the issuance of the OEC. However, the employment contract executed
between me and my foreign employer was not submitted to the office of the
Philippine Overseas Employment Administration (POEA). Also the same was left
by me in Kuwait. Hence, the absence of POEA-approved employment contract on
my part;
3. In view thereof, I hereby request the Office of the POEA to issue me the OEC
even in the absence of POEA-approved employment contract on my part. With
then issuance of said OEC, I hereby hold the Office of the POEA and other
government agencies free from any liability and obligation that may arise with the
said issuance of said OEC in my favor even without my employment contract;
4. I hereby further WAIVE whatever rights I may have against the Office of the
POEA and other concerned government agencies as a consequence of the
issuance of said OEC in my favor;
5. I hereby CERTIFY that the absence of the POEA-approved employment contract
on my part was not made for any fraudulent or illegal purpose/s;
6. I am executing this Affidavit to attest to the truth of all the foregoing for whatever
legal purposes it may serve.
