NCP FOR REPORTING

Nursing Diagnosis #3: Self-Care Deficit related to general body weakness, as evidenced by
dependence on assistance from therapist or family members for completing basic self-care tasks.

Goal: Within 8 hours of nursing intervention, the patient will demonstrate an improvement in
performing activities of daily living independently within a specified timeframe.

Expected Outcome:
Within 8 hours, the patient will:
a. Verbalized understanding of techniques and strategies to conserve energy and
manage fatigue.
b. Demonstrate improved strength and endurance as evidenced by increased
participation in self-care activities.

Interventions: Evaluation:

Independent: 1. The patient may demonstrate increased

Collaborative:
1. Involve occupational therapy for
specialized assessment and interventions
aimed at promoting independence in
ADLs.

assessing functional abilities and providing
interventions to improve independence in daily
activities, including ADLs.)
