Nurse : hello, Good morning Ms

Patient : Good morning, nurse . I want to check my illness.

Nurse : Yes, come in please. I'm sorry, how how would you like me to call you?

Patient : Oh, you can call me Mrs. Annisa

Nurse : My name's Rahmi, I'm registered nurse, and I'll be looking after you today. What are
you feeling now?

Patient : Not too well nurse. I have a fever and since a few days ago I have diarrhea. I'm
afraid my condition will get worse

Nurse : Okay, don't worry Mrs. We will examine you. Before it, now I've got some questions
I need to ask you, and some admission card. It's a personal data for your admission. Are you
ready? Is it okay if we started now?

Patient : Yes, of course nurse.

Nurse : Could you tell me your complete name?

Patient : My name is Annisa Permatasari

Nurse : Oh it's a beautiful name Mrs.

Patient : Yes, that's right nurse

Nurse : Could you tell me your nickname?

Patient : My nickname is Annisa

Nurse : How old are you?

Patient : I'm 65 years old

Nurse : okay, are you married?

Patient : Yes, of course, i have two children and perfect husband

Nurse : You have a happy family right?

Patient : Yes, everyday we feel happy

Nurse : Where do you live?


Patient : I live in Badung at Dalung permai number 13

Nurse : Oh same with me Mrs. I live at dalung too.

Patient : Really?

Nurse : Yes. And then what's your religion?

Patient : I'm a islam nurse

Nurse : What's your occupation?

Patient : My occupation is housewife

Nurse : When were you born?

Patient : I was born in Bandung on 5th August 1956

Nurse : you have a health assurance?

Patient : of course i have bpjs card

Nurse : Now, who's your family doctor?

Patient : My family doctor is dr. Stefan

Nurse : How many brothers and sisters do you have?

Patient : I have one brothers and two sisters

MEDICAL HISTORY

Nurse : How long have you had fever and diarrhea, maam?

Patient : for the fever just yesterday and diarrhea for about 3 days

Nurse : Okay right, so how many times do you poop in a day lately?

Patient : its like 4 or 5 times a day, also the consistency is little bit slushy

Nurse: Okay good, then can you give me a few details about your medical history?

Patient : I have AD, diabetes mellitus (DM), hypertension (HTN), and chronic kidney
disease. and previously I was diagnosed with Clostridioides difficile (CDI) infection based on
a positive stool test

Nurse : And what’s the reason you’ve come into the hospital today?
Patient : I had a fever since yesterday so I tried to take febrifuge, but my fever hasn't gone0
down until today. Also I had diarrhea for a few days,

Nurse : Are you taking any medications? And are you allergic to any drugs or foods? And do
you smoke or drink?

Patient : I am taking linagliptin (5 mg/qd) for DM treatment (admission HbA1c 6.3) and a
calcium channel blocker (10 mg/qd) for HTN treatment. 5 years before I was prescribed
donepezil (10 mg/qd) for AD. After being diagnosed with CDI, I took vancomycin (oral, 125
mg/qid) and metronidazole (oral, 500 mg/tid) for 10 days but to no avail. I’m not allergic to
anything. I don't smoke and I don't drink alcohol.

Nurse : Are you in any private health fund?

Patient : No, we’re not in any health fund.

Nurse : I’ll just take your temperature, blood pressure, blood sugar, physical examination like
weight, blood sample, and others. And then get you to write down the phone number of your
next of kin.

The doctor will come and see you in a few minutes.

Patient : Thank you.

Nurse : You’re welcome
