DEFINITIONS
SAFETY & HEALTH WARNINGS
1. The Purpose of the Lockout System is to ensure the safety of all persons working on equipment by effectively
disconnecting all possible sources of energy and/or harmful substances.
2. No person/s shall set a machine or machinery in motion or work on any machinery unless he has taken all
reasonable precautions to ensure that setting the machine in motion cannot injure him or other person.
3. No person/s shall conduct maintenance or repair work and no person shall cause or permit such work to be done until
equipment or machinery has been made safe by locking out electrically or mechanically.
4. Provision must be made for more than one person to lockout the equipment when more than one person is working
on that equipment.
5. The isolation must be such that remote or automatic start-up will be rendered ineffective.
6. A warning sign must be posted after isolation.
7. When working on ring feeds, isolation must be on either side of the work area.
8. Where equipment is sequence interlocked the immediate equipment before and after the equipment to be worked on
must also be isolated.
9. The isolated plant must be tested to ensure that the equipment to be worked on is in fact de- energized.
10. If the equipment cannot be locked out at the point of isolation, then the supply cable must be disconnected.
11. Pipelines must be isolated by chaining and padlocking the isolating valve.
12. When working on air or water lines mains, ensure that the pumps/compressor have been electrically isolated.

13. When repairs have to be conducted on secondary lines ensure that the line is isolated by means of gate valves that
branch off the main lines. These gate valves are to be locked out by placing a chain through the spindle wheel and
locking it with a padlock
14. When working on a hydraulic or pneumatic system ensures that the system has been electrically isolated and all
stored energy released.
15. When working on chutes that have flopper gates ensure that the flopper gate is isolated prior to commencing work
16. NB. Make sure that all stored energy is neutralized in the electrical, Hydraulic, and mechanical systems, prior to the
commencement of any repairs, maintenance or installation.
17. LOCKOUT CONTROL REGISTER
a. The supervisor or the artisans that is in charge of the job must contact the Site Safety Officer. Who will issue a lock
and key to the person/s in charge of working on the equipment. The said person must enter the relevant details into the
Lockout Control
b. Each person in charge must clear the Lockout Control Register, when the job is completed, by entering the following:
i. Date
ii. Time
iii. Area Safe Yes/No
iv. Signature
c. The same person who completed the register must clear the register.
18. PERMIT TO WORK
a. No work must be carried out or be allowed to be carried out on any equipment where there is or the possibility
exist that there is stored energy, for example executing tie-ins to existing operating plant; working near live equipment;
start-up and commissioning of electrical equipment and electrically driven machinery; working on live pipelines, confined
spaces and hydraulic equipment, unless such equipment, apparatus or cable is fully isolated in accordance with these
isolation procedures and a "Permit to Work" has been correctly completed and authorised by the owner of the
equipment Senior Representative
b. In the case of KRUGER premises the 16(2) assignee or in his absence the Safety Officer will be deemed to be
the owner of the equipment.
c. Once authority has been granted the equipment is to be isolated and the lockout is to be placed into effect in
the presence of the person authorizing the Permit to Work the key is
to be kept by the person in charge of the work
d. In the case of more than one team is carrying out different tasks on the same equipment each person in charge
shall lockout with his own padlock using a "Multilock" for this purpose and have a permit to work duly authorized.
e. All locks must have a tag with the date, name and contact no. of the person in charge of each team on it is to be
displayed and held in position by the padlock
f. The isolated circuit must be checked and tested both electrically and physically to ensure
no mistake has been made.
g. The person in charge of the work is also to complete a Workmen's Declaration to ensure that his whole team is
aware that a lockout is in place.
h. Each and every person who is to perform work on the isolated equipment must be shown the method of
isolation and that the equipment is locked out and the person in charge has posted danger notices. They must be
shown the limits of the isolation and be made fully aware that all other equipment must be considered 'LIVE" and
"DANGEROUS".
i. Only at this stage can the isolation be considered complete. Work may now commence but only on the
apparatus covered by the permit. Where emergency stop buttons or pull cords are installed, these must also be
activated.
j. On completion of the work, all persons must inspect the area to ensure all tools and equipment have been removed
and that it is safe to switch on before their personal locks may be removed and the isolators switched to the "ON"
position. Each person in charge must notify the person who authorised the Permit to Work The person in charge must
have the "Permit to Work" signed off by the said person.

k. The person in charge is then to return the lock and key to the Site Safety Officer and the
Lockout Control Register signed off as set out above. He is also to ensure that the Workmen's Declaration is to be
signed off by his whole team.
1. The person responsible for restoring the power must ensure that all loose items and tools have been removed
to a safe place and that all electrical equipment that may have been worked on has been correctly closed and that no
persons are in a position where they may be injured in the case of a startup.
m. Ample warning must be given that the power is to be restored.
n. Any switching when isolating or restoring power must be carried out in a smooth positive and swift manner and
not hesitantly.
o. Where it is not possible to use the lockout devices, the circuit breaker must be switched off and the outgoing
wires must be disconnected from the circuit breaker. The cable ends should be insulated with Scotch 33 PVC insulation
tape and danger boards displayed in a permanent position.
p. This system can only be used if the power to the equipment can be effectively isolated to perform the work
required (fans, winches, blowers, cranes, compressors etc.)
q. Only competent persons who are skilled electricians and have been authorised in writing by the owner of the
equipment's Senior Representative will be authorised to perform this type of isolation lockouts.
19. ISOLATION/LOCKOUT FOR DIESEL OR PETROL VEHICLES
a. The Lockout procedure must be carried out as follows:
i. Park brake is to be applied.
ii. Test park brake if possible
iii. Brakes must hold machine
iv. Place gear shift selector on neutral
v. Idle machine for 5 minutes if possible
vi. Stop engine
vii. Disconnect main battery if possible and practical.
viii. Display "Do not start" board on the window obstructing a person's view out of the vehicle.
ix. If available lockout machine at lockout mechanism using a multi-lock and your personal lock all persons working
on the machine must fit their personal lock to the multi-lock
x. Only persons working on the equipment are allowed in the cab during repairs or scheduled work
xi. Responsible person in charge of work must ensure that the vehicle key has been removed from the ignition
switch.
xii. Have at least one fire extinguisher available close to the machine in case of emergencies.
xiii. Fit locks to center articulation where provisions are made for this.
xiv. Any type of moving vehicles being worked on will have the wheels choked (blocked) on the both sides of a
wheel.
xv. If the vehicle brakes down on the road. Ensure that the vehicle is moved to side of the road.
xvi. Do walk around checks to ensure everything is in order
xvii. Demarcate the vehicle by using triangles or cones. Demarcation must be such that it would effectively guide
other road users
xviii. After completion of work, the responsible person is to ensure that all persons have completed their work, have
removed all tools and equipment and it is safe to operate the machine. The lockout padlocks and "Do Not Start" notice
board may then be removed and the machine started.
xix. Details of the repairs carried out logged in the Mechanical Equipment Repairs Log Book with the person who
did the repairs name, description of repairs and signature
20. LOSS OF KEYS
a. Loss of key(s) shall be immediately reported to the Site Safety Officer. Permission will have to be obtained to bypass
the safety device by filling in a "Permit to Bypass Safety Devices/Circuits".
21. SUB CONTRACTORS
a. All sub-contractors are to follow all the procedures that are contained in this SOP.
b. If the sub-contractor has his own lock it is advisable that they use such
.
22. GENERAL
a. In any situation where the above procedures are not practicable or relevant, then the principles used in the

lockout procedure should be adhered to.
b. These lockout procedures must be adhered to at all times.
c. The golden rule for lockouts should always be applied ISOLATES, LOCKOUT AND TEST.
d. The person carrying out an isolation and lockout shall only use the padlock supplied to him for this purpose and
the key for the padlock shall remain in his possession at all times.
e. When working on equipment that is plugged into socket plugs. The plug must be removed from the socket and
locked in the lockout box.
f Ladders used for servicing overhead cranes must be provided with a padlock and horizontally hinged door at
the bottom of the ladder to prevent unauthorized access.
