It has come to the attention of this office that there is was a possible data breach on the School’s
Enrollment System and Database.

We request the following from management:

A. That a composite internal investigation team be convened to investigate this matter

1. Joshua Ines– DPO;

2. Atty. Mark Cristian P. Dote – LEGAL;

3. Edwin E. Santos – IT;

4. Glenn C. Genoguin – SDI;

5. Alvin B. Rodriguez – Operations

6. Orly B. Cruz- Audit; and

7. Diane Jardin – Acad

B. To allow the team to secure third party assistance from IT Security Companies and our hosting site
AWS

C. To immediately shut-down the system to allow for the investigation

The objectvive is to:

a. Determine the cause of the breach and the possible extent of the breach, if any.

b. Protect and safeguard the data of our stakeholders from any damage
