CROA NOTICE
The following disclosure is being provided to you in accordance with the Credit Repair Organization
Act of 1996, as amended (the “Act”). Please note that GreenPath, Inc. does not consider itself a credit
repair organization, as defined in the Act. Please review this disclosure and sign it below to
acknowledge receiving a copy of the disclosure.
You have the right to dispute inaccurate information in your credit report by contacting the credit bureau
directly. However, neither you nor any “credit repair” company or credit repair organization has the right to
have accurate, current and verifiable information removed from your credit report. The credit bureau must
remove accurate, negative information from your report only if it is over 7 years old. Bankruptcy information
can be reported for 10 years.
You have the right to obtain a copy of your credit report from a credit bureau. You may be charged a
reasonable fee. There is no fee, however, if you have been turned down for credit, employment, insurance, or a
rental dwelling because of information in your credit report within the preceding 60 days. The credit bureau
must provide someone to help you interpret the information in your credit file. You are entitled to receive a free
copy of your credit report if you are unemployed and intend to apply for employment in the next 60 days, if you
are a recipient of public welfare assistance, or you have reason to believe that there is inaccurate information in
your credit report due to fraud.
You have the right to sue a credit repair organization that violates the Credit Repair Organization Act. This
law prohibits deceptive practices by credit repair organizations.
You have the right to cancel your contract with any credit repair organization for any reason within 3
business days from the date you signed it.
Credit bureaus are required to follow reasonable procedures to ensure that the information they report is
accurate. However, mistakes may occur.
You may, on your own, notify a credit bureau in writing that you dispute the accuracy of information in your
credit file. The credit bureau must then reinvestigate and modify or remove inaccurate or incomplete
information. The credit bureau may not charge any fee for this service. Any pertinent information and copies of
all documents you have concerning an error should be given to the credit bureau.
If the credit bureau’s reinvestigation does not resolve the dispute to your satisfaction, you may send a brief
statement to the credit bureau, to be kept in your file, explaining why you think the record is inaccurate. The
credit bureau must include a summary of your statement about disputed information with any report it issues
about you.

I hereby retain GreenPath, Inc. to assist in the repayment of my debts. All associated attachments including Interview Summary,
Income Statement, Home Budget, and Debt List pages are incorporated herein as part of this agreement by reference. This
agreement becomes effective on the date that I sign it and make a payment to GreenPath, Inc.
My total debt included in my DMP is $24,378.00. This debt is comprised of $0.00 of non-consumer debt which includes
housing and utilities, $0.00 of fixed consumer debt which includes all loans and leases, and $24,378.00 of unsecured consumer
debt. Based on the information I have provided and the current creditor policies for assessing interest, it is estimated that it will
take 54 months to liquidate my unsecured debt of $24,378.00 and additional interest of approximately $4,672.28 for a total of
$29,050.28. This estimate assumes, but does not guarantee that I will receive concessions from those creditors that offer them.
1. The term of this agreement is 60 months and will terminate on 8/9/2028. Repayment schedules that require more than 60 months
to complete may be renewed at 60-month intervals or less.
2. I agree that any monies paid to GreenPath, Inc. are for the purpose of payment to my creditors. I will pay a one-time set up fee of
$0.00 for consultation, negotiations with my creditors, setting up my account and the like. I will pay a one-time fee of $0.00 to
purchase Credit When Credit Is Due. I will pay a fee of $52.04 per calendar month or any portion thereof as a service charge for
the maintenance of my debt management program including, but not limited to, the disbursement of payments to my creditors,
communicating with my creditors regarding the status of my account and making changes to my account as necessary to ensure
that is is administered in my best interest. The total of fees to be charged during the term of this agreement will not exceed
$3,122.40.
3. I agree to make deposits to GreenPath, Inc. in the following manner: My first deposit to GreenPath, Inc. will be paid on
8/10/2023 in the amount of $52.04. All future deposits will be as follows:
5. I fully realize that it is my obligation to see that the agreed-upon deposit reaches GreenPath, Inc. in a timely manner. I understand
that if I do not consistently remit the specified amount at the specified times, my debt may not be reduced and this agreement
may be cancelled.
6. I agree that GreenPath, Inc. is not responsible for any delay in payments or missed payments to creditors caused by GreenPath,
Inc. not receiving the deposit as specified in Paragraph 3, regardless of the reason for that delay.
7. I have disclosed all credit/charge accounts and I am fully aware that the creditor(s) has the right to close the account(s). I agree to
destroy all credit cards associated with the lines of credit on the Debt List. I agree not to open additional credit accounts or make
new credit purchases while on the DMP without notifying GreenPath, Inc.
8. I understand that GreenPath, Inc., acting as my agent in negotiating arrangements with my creditors, is free to exercise its own
judgment in making such arrangements; may provide full financial information to creditors involved in the arrangements; and
shall not be responsible or liable for any action taken by my creditors. I understand that creditors may assess late fees and/or
increase interest rates if payments are not received in a timely manner.
9. I further understand that GreenPath, Inc. is subject to periodic review by governmental/regulatory agencies and third party
auditing, licensing, and accrediting organizations and that such reviews may entail inspection of client files and release of client
information. In addition, I understand that client information may be provided to third party suppliers and other third party
agencies, at the discretion of GreenPath, Inc., as deemed necessary in order to properly manage client accounts.

10. I understand that either GreenPath, Inc. or I may cancel this agreement at any time upon written notice to the other party. In the
absence of written notice this agreement will remain in effect and monthly fees will continue to accrue. This agreement will be
canceled automatically if I fail to make a deposit within 60 days of my deposit due date as specified in paragraph 3. I understand
that my monthly maintenance fee will cease upon cancellation of this agreement.
11. I understand that I am entitled to a refund of all fees paid if I cancel this agreement within 3 business days of signing it. I
understand that I must provide written notice of my desire to cancel this agreement to GreenPath, Inc. by midnight of the fifth
business day.
12. I understand that GreenPath, Inc. may hold funds in my debt management account in order to disburse payments in my best
interest. I will review my statements from GreenPath, Inc. and statements from my creditors to see how my funds were
disbursed. I will contact GreenPath, Inc. with any questions.
13. I understand that many factors influence my credit report and credit score and I acknowledge that GreenPath, Inc. does not
represent or warrant that my credit score or worthiness will improve by entering into a DMP. I further acknowledge that creditor
concessions are all subject to individual creditor policies and that GreenPath, Inc. is not responsible for differences in creditor
policies and makes no guarantees regarding creditor concessions including payment, interest rate, fee, re-age or other concessions
that creditors may agree to.
14. I understand that the goal of the DMP is to assist me in developing creditworthy behavior and paying my debts which may result
in improved actual credit as a collateral consequence, rather than as a program objective. I further understand that GreenPath,
Inc. does not consider itself a Credit Repair Organization, as defined in the Credit Repair Organization Act of 1996, as amended.
15. I fully understand that this written agreement represents my entire agreement with GreenPath, Inc. I further understand that any
future agreements between GreenPath, Inc. and me will be of no effect unless such agreements are in writing and signed by a
duly authorized representative of GreenPath, Inc. and me.
16. I acknowledge that I have read this agreement completely, I understand it, and I have received a signed copy of this agreement.
You may cancel this contract without penalty or obligation at any time before midnight of the 3rd business day after the
date on which you signed the contract. See the attached Notice of Cancellation Form for an explanation of this right.

GreenPath has been helping people with financial problems since 1961. Our process is simple. We will do a budget analysis that will examine
your financial situation, discuss the factors that may be the cause of your problems, and explore your options for developing a reasonable plan
for dealing with them. GreenPath is a member of the National Foundation for Credit Counseling (NFCC). The NFCC has high standards for
quality credit counseling and financial education, and this agency complies with those standards. In addition, GreenPath is a HUD approved
housing counseling agency. We are accredited by the Council on Accreditation (COA), an independent third-party organization that reviews
and monitors entities that provide services to families. We are a non-profit agency that operates in accordance with Section 501(c)(3) of the
Internal Revenue Code.
Your privacy is important to us. We make certain that all information shared orally, electronically, and in writing will be managed
within legal and ethical considerations. We will not disclose any of your personal information to third parties without your consent.
We will take precautions to disclose information only to those who have a right to know. A copy of our entire privacy policy is
available upon request or can be accessed from our website.
Please read the following statements carefully so that you will understand the procedures for the counseling session.
. I understand GreenPath Financial Wellness (GreenPath) will provide a confidential financial assessment during which a qualified certified
credit counselor will complete the assessment. The counselor will review all action plans developed.
. I understand that I may accept or reject any part of the written assessment. The assessment may include the following options:
a) I will handle any financial concerns on my own.
b) I will choose to enroll in GreenPath’s Debt Management Plan (DMP). The DMP serves the dual role of helping you repay your
debts and helping creditors receive the monies owed to them.
c) My counselor may answer questions about bankruptcy, but not give legal advice. I will contact an attorney for legal advice. While an
attorney can make a recommendation to file bankruptcy, it is a personal choice based on individual circumstances. I will inform
GreenPath if I decide to file bankruptcy.
d) A counselor may refer me to other services as appropriate. These services may be able to assist with particular issues that have been
identified.
. GreenPath is approved by HUD to provide the following housing counseling services: mortgage delinquency/default resolution,
pre-purchase, rental, and reverse mortgage counseling. I understand that I am not obligated to receive, purchase or utilize any other
services offered by GreenPath in order to receive housing counseling services.
. GreenPath will not obtain a copy of my credit report without my consent.
. GreenPath will not inform any credit-reporting agency of my participation in the DMP. GreenPath has no responsibility or obligation for
any past, present, or future credit rating I receive. In certain circumstances, a DMP may negatively affect my credit rating. In the event
that the counselor suggests a DMP, I will receive complete details of the DMP, its requirements, and the responsibilities of GreenPath
and myself.
. I understand that a portion of GreenPath's funding comes from creditors participating in DMPs. Since creditors have a financial interest in
getting paid, some are willing to make a contribution to help fund GreenPath. These contributions are usually calculated as a percentage of
payments you make through your DMP, up to 15%. GreenPath will work with all creditors regardless of whether they contribute.
. All fees for services will be disclosed and payment methods agreed upon prior to delivery of services.
. I understand that if I have a concern regarding the services I have received, I may contact any member of the team with whom I am
working. A copy of the Customer Concern Process will be provided to me upon request, should it become necessary to file a formal
