Investigative Report on break fire in Packaging Unit.

Subject: Investigation report on fire break in the packaging unit
Introduction:
Purpose:
The purpose of this report is to represent the findings of the investigation
carried out regarding the massive fire breakout which occurred on 22 nd of June
2022, in the packaging unit of XYZ Biscuits causing major loss to packaging
equipment and huge quantity of biscuit boxes. The investigation was
authorized HSE manager to know the background facts that caused fire in the
lab.
Personnel:
Following people were included in the investigation
1. Manufacturing supervisor
2. Laborer
3. Packaging unit head
4. Quality assurance engineer
Discussion:
The investigation committee looked deep into the matter and considered every
possibility that might have caused the fire. There were 500 boxes of Biscuits, 3
conveyer belt robots and 1 quality checking machine in the factory which are
now destroyed. The wire did rise in massive amount and was controlled after 5
hours by much struggle of fire brigade in vicinity of the incident. The number of
biscuit boxes and machines in each lab are given below

The team looked to into the details of the damage done by the fire and
concluded that all the machinery that was available in the unit along with
biscuits were destroyed and could not be used.
Procedure:
The team investigated each unit and looked for the burns and destroy of
equipment. The team looked in all the possible reasons that cause a fire
breakout such as all electric wires going into the packing unit our places where
there are high temperature requirements.
The team used the technical methods to find out if there was a circuit
break that caused the fire.
The team interviewed the staff who were duty at the time of accident.
Then they did a brief examination of the surroundings of the unit
building to check out for any fire sources that could travel into the
packing unit.
Lastly, the team looked to the electricity board so that if there might
have been an issue in the circuit board. The team found that there was
an issue in circuit board
Result:
After the above procedure it was confirmed that the fire had broke out due to
an issue in the circuit board which caused the fire breakout.
Conclusion/Recommendations:
The team found out that the quality material of wires and circuit board was so
poor which could not prevent such enormous accident to happen. The fuses
were also cheap and of poor material which should not have been used in the
circuit board of these labs.
It is recommended that wires of good material be used in the electrical
circuitry.

Theft of Expensive Equipment’s Incident Report
INTRODUCTION:
I am doing job in Mobile Phone Market as mobile seller and last Friday a very unfortunate
incident happened. I had fallen a victim to theft that was committed on June 10, 2022, in the
Local bus.
PURPOSE:
On June 10, 2022, at around 4.30 PM, the theft stole the money and expensive equipment in
the Local bus.
PERSONNEL:
About 30 passengers travelling in the bus and the theft stole the money and expensive
equipment due to no security in the local bus.
AUTHORIZATION:
To avoid such incidents in the future, the report is written as the S.H.O demanded it.
DISCUSSION:
On Friday, after I leave the Bank almost 4:00 PM I went to the Local bus station
near Bank office. When I enter the bus, I have bag which contain mine office
Laptop, official documents and wallet. I properly checked my bag. But after second
last station when I left the seat, I am wondering where my bag is which usually I put
on side of my seat but there is no bag around me.
Someone took this as an opportunity and took my all money. I had to pay for my
Shop expanses that’s why I had withdrawn the whole salary. Otherwise, I withdraw
money according to my needs. I had thought to take it along with me when I will go
back home.
FINANCIAL DAMAGE:
I had huge financial damage because laptop is very expensive. Someone has stolen the
salary that I had withdrawn from the bank. My laptop worth of 70 thousand rupees and
wallet with cash money of almost 3 lakhs were stolen from Local bus.
EQUIPMENT DAMAGE:
My identity card, ATM card, Shop documents and some of important bill’s receipts were
also present in the wallet and laptop. They are very important for me, and I must make a
very long process for their re-issuance.
PERSONAL DAMAGE:
Sir, all the shop data and shop documents were present in it. Moreover, my family pictures
and some personal information were also in it. I have lost very important official and
personal data of my shop which is in Laptop.
CONCLUSION:
This letter is meant to request you about investigating this matter and bring the criminal on
the screen. It is my humble request to you to please have a look at this matter and let all of
us know the true criminal. I cannot afford to buy a new laptop and mobile phone with my
existing salary. This incident can happen with anyone in the bus next time if something not
seriously taken in this regard.
RECOMMENDATION:
Strict action against culprits should be taken:
I want you to look upon this matter and bring the culprit to the audience. You should find
the group or person that is involved in wrongdoing. This time I have become the victim of a
theft incident. Next time it can happen with any of the passenger which is traveling during
that interval of time.
Installation of security cameras:
And it is my humble request to please install security cameras inside and outside of the buses,
as they can be very helpful in such unfortunate incidents.
Strict Investigation:
To minimize the frequency of these types of incidents, a very strict and indifferent or neutral
investigation is very necessary.
Therefore, I request you to take kind consideration to my report and do something in this
regard.
