BACKGROUND
Family Background
- The subject’s family is composed of the parents and five siblings
wherein the subject is the youngest child, four of the sibling has its own
family and one of them is separated to them for he has its own family.
Due to the huge gap of age among the siblings, every one of them
experience wellness of the parent, but if a new child was born the
parents focus is in on the new one, and older sibling will become
independent to the needs and help from their parents. Subject
experience lack of guidance and wellness in her high school on
financial needs. Parents are strict but still well on subject. From its
childhood to senior high school day parents are the one who takes
care of her, on its college her mother and boyfriend are who helps her
in need. Her relationship with her boyfriend is well considerate, give
and take is also seen for the boyfriend provides for her, while the
boyfriend do the work while studying.
Medical History
- No current medical conditions, last check-up is since high school
where there is an abnormality on nerves on its head, Urinary Tract
Infection, and headaches was also experienced in its high school. On
its elementary, furuncle exist and sees on the hand and gets insecure
on what it looks like but when heal, insecurity is gone. With further
injuries there is no more such thing was experienced. No medical
health conditions, and substance abuse. The most experienced stress
happens when its father died that gets to anxiety, wherein she isolates
herself in house and when gone to work suddenly forgets its password
on its phone and until now still forgotten. Experience blankness of mind
where she only remembers that he goes out the store where she works
from the past and walks to the place that did not know and
unconscious to where she is going. Consciousness comes back when
a friend of hers greets her and met in its walk. Weight loss is
experienced when its father died, that also affects its sleep pattern and
changes its body clock. In coping up stress, when experiencing stress,
she tends to talk with its friends, do household chores, and watch
psychology facts videos.
Education History
- The only problem that affects her academic performance is her
financial need for its economical status and financial problem. When
there is an upcoming exam, she tends to make herself happy which
she said to be what makes her more confident to answer and make her
less stress to take an exam. Her goal after graduating college is to
work already to overcome financial needs and provides for the family
and herself, for she knows that this is the problem she is always facing
on its needs.
MSE (Mental Status Examination)
Jenisa appeared his stated age 22. She was noted to be well-groomed
ectomorph body in white psychology uniform of the college. Answers question openly
using gestures while talking, calm, relaxed, cooperative answering questions. Does
making eye contact to its interviewer. Euthymic, yet when talking about stress and her
anxiety experience from the past due to the loss of his father, her voice goes low and
gets slow while talking and sharing her past experiences. The way of thinking is very
sequential and goal-directed, giving explanation to its answer making it clear. The
subject make sense of its answers showing its independence and caring for her loved
ones. There is no sign of mental illness or any issue to be aware of.
BEHAVIORAL OBSERVATION
Jenisa’s action and words give accurancy on what its feelings that is observed
under the interview, she often touches her nose when topic or a statement to that is
about to be shared is personal to her. Talking about the grievance of the loss of the
father make her voice and hands that is making gesture shake a bit showing
temperance of its emotion toward the topic. Fixing of sitting position and posture is
always seen before and after answering question, that implicates of willingness or
openness to give information that is needed for the report.
TEST ADMINISTERED
The test that is given to the subject is composed of four test; the House-Tree-
Person test, Sacks’ Sentence Completion Test, Bender Visual-Motor Test, and Filipino
Intelligence Test. The test is administered in a classroom with the guidance of the
subject teacher of Psychological Assessment. All what is need to start each test is done,
the time of each test done is observed and noted to give assessment how is the
cognition of the brain do its work, and so the environment is quite not secured for there
are so many distractions, but making sure that the test taker is focused on its own paper
is done.

House presents the situation of the person on its domestic side. The roof shows
the high aspiration of dreams, which signifies hope and effort to achieve them. Light

Details that are written down is well seen in the family background showing
disconnectedness yet independency having stable emotional state from what have been

In overall interpretation of the tree, it shows the subject towards it’s feeling ang
role in life, shows the personality of wanting and willingness to give care to self and

The head drawn last is interpreted as a sign of severe thought disturbance. This

shows independency of the subject to live situation, yet shows stress at disturbances
remembering of loss of someone special. Family bonding is not strong with its sibling,

that give guilt when the decision is not well, which may become a memory that can be
remembered so many times. Confidence is present but that strong enough to give
satisfaction for the well-being of subject.

INTERPRETATION
The subject’s well-being made by its parent causes dependency on her, but as
soon it grows up his goal to take care of her family and to build her better life and
establishing a good wellness of life in the future. The test shows normal state of mind.
The intelligence test shows a score of below average result. The other test shows like
the Bender Visual-Motor Test, shows the normal flow and control of her thought and
emotion as what is written on its background where she tends to control her emotion for
her goal. The Sacks’ Sentence Completion Test Shows normal state of life and well-
being, yet facing challenges in life gives her disturbances that appears on his family
background that causes her stress and anxiety. The House-Tree-Person indeed shows
his role and identity towards the drawing of his parents, tree shows his roles towards its
surrounding bearing a fruit which gives preservation and giving life or care to others,
and the house that show her situation in its domestic area as well as the details that is
seen on it.
SUMMARY
Based on her performance and answers to the questions that is asked, it shows
normal state of mind and yet hard to face problems, making decisions, and feel guilt
what has left behind in the past. Communicating to the subject shows her openness to
the topic and answering the question and so when talking about personal experience
changes of its tone, volume, mood, and even its behavior seen to be expressive.
Referral questions indeed gives better than expected number of information that will be
gathered, and yet gives very useful informations for the assessment and the MSE
(Mental Status Examination).
CONCLUSION
All information concludes that the subject makes sense of every information that
is given that it all shows accuracy that the subject is indeed dependent on his life, yet
gives and take her for her loved one that is there for her, experience anxiety and
unconsciousness of actions and sudden memory loss of a specific matter when in
experiencing grief. The guilt of not making decisions and making not good decisions on
her life remains to her as she not forget what happened form the past that needs to give
attention.
RECOMMENDATION
According to the reviewed evidence, Jenisa has difficulty on moving forward and
moving on what’s on the past. It also shows that when experiencing grievance, a part of
a memory is lost and do not remember it as what it is and gives unconscious actions
when inside of the thought. It is recommend to make the subject help her feel that she is
not alone and people surrounds her can be a crying shoulder for her to express the
emotion that is suppressed inside her to make her feel more better.
Attending to a group interventions is also recommended, for her to make her
know that there is also someone that experienced challenges like hers, and so to give
her other insights from other people how they face there challenges in the situation they
are in. Online group interventions is indeed recommended if busy there is no time to go
out, or busy inside the house doing chores. Watching inspiring and funny videos, can be
recommended also, making herself entertained and well inspired gives happiness and
positive thinking that avoids her to negative thinking and thoughts.
