1. I have appeared S.S.C.Public Examinations held in the month of

2. I submit that my parents are illiterates. That at the time of my admission in

the School, my father has got mentioned my date of birth/name as ____________
instead of________________. I submit that the mis-recording of my date of
birth/name is caused due to over-sight, hence the same deserves to be corrected.
If my date of birth is not corrected in the Secondary School Certificate I will be put
to irreparable loss, which cannot be compensated in terms of money.

3. Hence, this sworn affidavit with a request to kindly correct my date of

Secondary School Certificate.
