Observations: An unannounced visit was made to this center following attempts to call the center and receiving a
"disconnected" message.
The visit was conducted with the owner/ operator of the center.
Fire inspector, Robert Carmon was on site conducting a visit with an individual who has stated that she is assuming
ownership of the center effective July 1, 2007.
The current owner/ operator of the center provided for me copies of the letters of intent to sell and the intent to purchase
that were said to have been mailed to Lead Consultant Cora Revel. The letters were dated May 23, 2007.
The owner stated that the phone number has been changed due to the pending change of ownership. The new number is
(336) 271 2725. Only local calls can be made from this number which does not include calls to Winston-Salem. I
explained the center must be able to contact parents of all children enrolled in the event of an incident or emergency.
The current owner stated that she has decided to sell the business due to personal/ family reasons.
She stated that she and her husband do not intend to remain in child care.
The individual who is planning to assume ownership of this center, stated that she is in the process of completing
inspections and will then contact Kay Lowrance of the Division of Child Development.
A change of ownership cannot occur until a Temporary license is issued from a Division representative. Kaye Lowrance
will assign a lead consultant to assist with the change of ownership.
During today's visit, seven violations were documented and reviewed with you as follows:

served as the protein and did not contain a sufficient amount of meat. This is a violation of a requirement in 10A NCAC
09 .0901(a).
1006 One or more persons who met the qualifications for administrator were not on site for number of hours per week
required by capacity of center. No one on site meets the qualifications of a Level 1 Administrator. This is a violation of
a requirement in .0714(a).
1013 At least 1 person who met qualifications for lead teacher was not responsible for each group for at least two-thirds of the
total daily hours of operation. There was not a staff person on site who is verified with the Division as meeting lead
teacher requirements. This is a violation of a requirement in 10A NCAC 09 .0714(c).
1041 Criminal Records Check Forms were not submitted within required timeframe or maintained in the employee's
personnel file. The owner and the husband of the owner have not been qualified by the Criminal Records Unit of the
Division of Child Development. This is a violation of a requirement in G.S. 110-90.2(b).
1043 All staff records, except financial records, were not made available for review. A file was not on site for a staff member
counted in the ratio during today's visit. This is a violation of a requirement in G.S. 110-91( 9).
1048 There was not the required number of staff present who had successfully completed a basic first aid course within the
last 3 years. Documentation was not on file verifying that someone on site had completed a basic first aid course. This
is a violation of a requirement in .1102(c).
1049 There was not at least one person who had successfully completed a course in CPR appropriate for the ages of children
in care on the premises at all times and/or accompanied the children whenever they are off the premises. Verification
was not on file that any one had completed a CPR course within the past year. This is a violation of a requirement in
.1102(d).
Comments: These violations must be corrected immediately and a signed letter mailed to me by July 5, 2007 describing

You, owner were reminded today that a proposed Administrative Action is underway against your license because of the
violation cited during a complaint investigation on 11/14/06 regarding attempts to falsify information regarding
Credential Certificates.
Files for the owner and the husband of the owner contain North Carolina Early Childhood Credential Certificates when
the Work Force Unit of the Division of Child Development does not have record of having issued credentials.
Additionally, Criminal Records Qualifying letters are on file for the owner and the husband of the owner. The Criminal
Records Unit of the Division does not have record of having ever received criminal records forms nor issuing qualifying
letters to you or your husband.
In a phone conversation with you on 4/20/07, you agreed to mail new criminal records forms to Raleigh and new official
transcripts and an application to apply for the Credential.
Today, you stated that you placed these items in the mail following the phone conversation, however they were never
received by mail and the criminal records system does not reflect that these items have been received.
As of today, neither you nor your husband still have not been verified to meet Administrator nor Lead Teacher
requirements. Also, neither you or your husband have been qualified by the criminal records unit.
Even though you have stated that you are in the midst of a change of ownership, these items are still due to be received in
my office within two weeks of today. Criminal records forms must be mailed to Raleigh with copies mailed to my
Winston-Salem address.
Official transcripts and an application to apply for the North Carolina Early Childhood Credential must also be received
in my office by July 5, 2007.
Someone who meets the requirements of an Administrator must be on site the required number of hours based on the
capacity of your center effective immediately.
If the operator fails to correct any documented violations within the established time period, the Division of Child
Development may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000).

Observations: An unannounced visit was made to this Center to monitor compliance with child care requirements during
your annual compliance visit.
Upon my arrival today, two caregivers were present with 18 children ages 0 to 4. Later, the co-owner/ group leader
arrived.
The co-owner assisted me with this visit in the absence of the director/owner.
Staff records were not available for review. You stated that they were locked in a file cabinet and only the director (who
was not present) has access to them.
Violations:

301 Staff/child ratios and group sizes for the number and ages of children in care were not met. Two caregivers were
responsible for 18 children ages 0 to 4. This is a violation of a requirement in GS 110-91(7);.0713(a-e).
303 Children were not supervised at all times as required by applicable rules. Upon my arrival to the Center, one caregiver
cared for children ages 0-2. The caregiver for ten 1 and 2 year old children who slept on the opposite side of the
Center was sitting outside on the porch talking with visitors. The ten 1 and 2 year old children could not be visually
supervised by the one caregviver in the room for infants. four year old children were sleeping across from the
space for infants in a space visible to the caregiver in the room for infants. Staff said the teacher for the t four year
old children had gone to pick up after school children. This is a violation of a requirement in .1801(a)(1-5).
428 The activity plan was not current or accessible for easy reference by parents and by caregivers. Activities plans for the
current week were not available. This is a violation of a requirement in GS 110-91(12); .0508(a).

812 Electrical outlets not in use were not covered or inaccessible. Electrical outlets in the room for one year old children
were left uncovered. This is a violation of a requirement in 10A NCAC 09 .0604(c).
841 All medicine was not kept in locked storage separate from food and cleaning supplies. Medication stored in the
refrigerator was not locked. This is a violation of a requirement in 15A NCAC 18A .2820(d).
849 Leftover medicines were not returned to the parent. Medication for a child who no longer attends the Center was stored
in the refrigerator. This is a violation of a requirement in .0803(12).
895 All infants under the age of 12 months were not placed on their backs for sleeping unless the center had obtained the
appropriate written waiver. A -old infant was placed on stomach to sleep. This is a violation of a
requirement in .0606(a)(1)(A-B).
1043 All staff records, except financial records, were not made available for review. Staff files were not available for review.
This is a violation of a requirement in G.S. 110-91( 9).
1115 First-aid kit was not located in vehicle; kit was not mounted or otherwise secured if in passenger area. First Aid
supplies were not located in the vehicle which was used by Center staff to transport preschool and school age children
from public schools to the Center. This is a violation of a requirement in 10A NCAC 09 .1003(c).
1116 Children's ID. and emergency information were not in vehicle. Emergency and identifying information for the children
being transported by Center staff from public school to the Center were not in the vehicle. This is a violation of a
requirement in 10A NCAC 09 .1003(d).

accessible at all times in the event of an emergency regarding a staff member.Today we discussed the requirement that all
infants be placed on their backs for sleeping unless a physician's waiver is on file for infants under 6 months, and a
parent's waiver for infants over 6 months.You stated that the vehicle being used to transport children has a disabled
passenger side airbag. Today I explained that children cannot be transported in the front passenger seat unless a written
document is received from the dealer verifying that the airbag is disabled.Your blocks and manipulatives are grouped
together. We discussed separating them to make two separate activity areas.You stated that the electrical outlets in the
Center were the safety type that does not require a cover. In the room for infants, I was able to see the safety device by
looking in the outlets. In the classroom for one year old children, the outlets appear to be standard. The outlets in the
room for one year old children must be covered when not in use.I can be reached at 748-4661 if you need

Children were observed having free play, group time, and outdoor play.
A lunch was served during my visit consisting of chicken nuggets, peaches, green beans, and milk.
A fire inspection was conducted in November 2006 by Inspector Robert Carmon. You agreed to mail the yellow form to
me.
A sanitation inspection was conducted on 7/14/06 by Environmental Health Specialist Paula Cox . You received 8
demerits and a "superior" rating.

405 A child's hands were not washed after each diaper change. Two children were observed not having their hands washed
with soap and running water immediately following a diaper change. This is a violation of a requirement in 15A NCAC
18A .2803(b).
427 A written plan of developmentally appropriate activities for each group of children in care was not available. An
activity plan was not available for the children in care. This is a violation of a requirement in GS 110-91(12); .0508(d)
(2).
501 Meals and snacks did not comply with the Meal Patterns for Children in Child Care Standards. Bread was not served
with lunch. This is a violation of a requirement in 10A NCAC 09 .0901(a).

615 Beds, cots and mats were not in good repair, properly handled, stored, or clean and sanitized between users. Mats were
stored so that the sleep side of one mat touched the floor side of another mat. This is a violation of a requirement in 15A
NCAC 18A .2821(a).
620 All walls and ceilings were not kept clean and in good repair. Sheetrock in the space for one-year-olds and infants is
chipped. This is a violation of a requirement in 15A NCAC 18A .2825(a).
805 A complete record of monthly fire drills showing the date, time, length of time to evacuate and signature was not
maintained. A fire drill was not recorded for the month of October 2006. This is a violation of a requirement in .0604(t);
.0302(d)(5).
859 Monthly playground inspections were not completed. Playground inspections forms were not available for 2006. The
last report observed during this visit was dated 12/16/05. This is a violation of a requirement in .0605(q).

1034 All staff did not have an annual medical statement or health questionnaire on file. The most recent health questionnaire
for one staff was dated 1/5/05/ This is a violation of a requirement in .0701(a).
1041 Criminal Records Check Forms were not submitted within required timeframe or maintained in the employee's
personnel file. Criminal records forms for a staff member who began work on Wednesday, November 1st have not been
submitted to Raleigh. This is a violation of a requirement in G.S. 110-90.2(b).
1043 All staff records, except financial records, were not made available for review. A teacher on duty during this visit did
not have a file on site. A new staff person hired 11/1/06 did not have an application and emergency information on file.

1052 Staff required to receive in-service training had not completed the required number of hours according to their
education, experience, accumulated training and hours of employment. Two staff did were missing 7 hours of in-

administrator is required to be on site at least 20 hours per week for centers licensed for less than 30 children. You stated
today that your wife, owner of this facility is now working a different job and that you are in charge of the facility. You
only meet lead teacher requirements.An administrator must be hired immediately. old child spent an excessive
amount of time in a high chair with no activity. This child must be given opportunities to play outside and to move about
the activity areas.The fire inspector completed a pink form but not the yellow fire inspection form required by the
Division of Child Development. You called Inspector Carmon during my visit and left a message requesting that he
come back out to complete the yellow form. You agreed to mail it to me right away.We discussed that staff should
interact with children while on the playground. Staff should not use cell phones while responsible for a group of children
except to contact someone in an emergency.You said you did not attend the training about the new rated license. New
child care rules were also reviewed during that meeting and new rule books were given to providers who attended. New
child care rules which went into effect January 1, 2006 were reviewed with you regarding in-service training, posting of
staff/ child ratios in every classroom, and notification to parents of substantiations of child abuse or neglect or the
issuance of administrative actions. A packet explaining the new rules was left with you. Due to the number of violations

Observations: An unannounced visit was made to this Center to follow up on violations regarding supervision and
staff/child ratios cited on 11/7/05 during your annual compliance visit. The assistant owner assisted me with this visit in
the absence of the owner. During today's visit, ratios were in compliance. No incidents regarding lack of supervision
were observed. You stated that an additional full-time staff person has been hired and will begin work on Thursday,
November 17th to correct the potential staff/child ratio and supervision issues.I reviewed staff/child ratio and supervision
requirements with you and with a newer staff member during this visit.Staff records were reviewed today.Two violations
