WHEREAS, the DONOR is the owner of an electric oxygen machine, which he/she
desires to donate to the DONEE;
WHEREAS, the DONEE is a duly recognized barangay government unit that is capable
of receiving donations and has the intention to use the donation for the benefit of its
constituents;
NOW, THEREFORE, for and in consideration of the mutual promises and covenants
hereinafter set forth, the DONOR voluntarily and freely gives, transfers, and conveys by
way of donation the following property to the DONEE, who accepts the donation:

2. Purpose of Donation:
o The electric oxygen machine is donated for the purpose of supporting the
healthcare needs of the residents of Barangay Sinense and may be used
in barangay health programs or emergency response initiatives.
3. Delivery of Property:
o The DONOR shall deliver the electric oxygen machine to the DONEE at
the Barangay Sinense Hall or any other location mutually agreed upon,
and such delivery shall constitute transfer of ownership.
4. No Conditions Attached:
o This donation is made without any conditions or restrictions. The DONEE
shall have full ownership, including the right to use or dispose of the
electric oxygen machine as it sees fit for the benefit of the barangay's
constituents.
5. Acceptance by the DONEE:
o The DONEE hereby accepts this donation and agrees to the terms and
conditions set forth herein.
