MAXIEM Installation Agreement
Dear Customer:
We would like to say “Thank you” for your recent purchase of the Maxiem Waterjet Machining
Center.
The following information is to be used as a guide and will provide you with a general understanding
of our installation process.
OMAX Responsibilities:
• Work with Customer assigned to the installation to resolve any utility and or site
matters during pre-installation interviews.
• Schedule dates of installation for a time that is suitable for the Customer as well as
OMAX Corporation.
• Arrange travel and accommodations for the OMAX FST.
• Supply Travel Itineraries: OMAX Technician’s or Technicians’ name/ names and Itinerary
will be sent via email.
• Our FST will arrive on-site promptly and on time every day. The expectation is that we
work from 8:00 AM to 5:00 PM every day. Overtime is okay and needs to be worked out
between the customer and our technician. The only exceptions are on our travel days.
(Mondays and Fridays)
• OMAX FST will be assigned to take pictures of the machine after it is completely
installed for our records.
• OMAX FST will help determine as to whether or not there are any further training needs
at the end of the installation process.

Week 1:

Day 1: Travel back to the worksite in most cases. Again, this may take all day and the technician will start
the following items Day 2:
• Finish Any Remaining Installation Tasks
• Begin Operator/ Maintenance Training (If it is a machine with extra accessories, this may still
be completed either Day 1 or Day 2).

Customer’s Responsibilities
• Site utilities must be ready prior to the OMAX FST being dispatched to provide installation service. This is
confirmed with our Installation Coordinators during the pre-installation interviews and finalized with a signed
copy of the Installation Agreement Form. Initials
• Prior to our arrival, it is necessary to confirm the floor or slab is level within 1 inch over the entire length
and width of your machine. Check the machine's spec sheet for measurements. Initials
• Customer must have lifting capabilities and hardware available to move and or position all equipment when
needed. Including and not limited to adequately sized forklifts/ cranes, lifting straps, spreader bars as well as
equipment operators or a rigging service on call. Initials
• Prior to our arrival, it is necessary to remove the catcher tank from the shipping skid and move it into the
designated installation area. Initials
• Secure Storage - Designate an area to store boxes and additional crated equipment and ensure that
everything remains untampered with until the OMAX Technician is on-site. Initials
• It is essential to have someone present at all times for system assembly assistance. Part of the training for
your personnel will be to know how the equipment goes together for future maintenance needs.

• If no one is available to assist, special arrangements will need to take place via OMAX Human Resources to
obtain a laborer from a local Temporary Service. This may cause a delay up to three weeks to work with a
local Temp Agency to screen people to obtain a qualified candidate. Initials
• Do Not misplace or lose any of the items in any of the cardboard boxes that have arrived with the machine
shipment. The OMAX Technician is going to perform an onsite inventory of all items on the original sales order.

followed to prevent damage to the equipment. Initials
• Please Do Not delay the installation process by assigning your personnel to do other things while they are in
the process of learning about your new OMAX Waterjet Machining System and operational procedures.

In order to efficiently and successfully install an OMAX Waterjet, OMAX requires that the utilities (air, water,
drain, and electrical power) be completed and ready for usage prior to the technician’s arrival on site.
I have reviewed the attached pre-installation checklist with an OMAX representative and understand the
responsibility to provide the resources to successfully install an OMAX system. Failure to provide the utilities
prior to the technician’s arrival will reduce the amount of time available to train the operation and
maintenance personnel as purchased with the machine. It is possible that additional charges (minimum
$8,000.00) will be incurred should another on-site visit be required to complete the standard installation or
training.
I understand and agree to the above requirements and authorize OMAX to arrange a technician to visit our
site to provide installation & training services. Failure to sign and return this form promptly will delay your
installation date.
