I, __________, the undersigned, hereby request you to kindly initiate Pre

Litigation Mediation with my spouse Ms. __________, aged about

1. I state that Ms. __________and I got married on __________at

__________, Bangalore, as per Hindu Rites and Rituals and we


have a child __________who was born from this wedlock on

__________and she is currently almost __________ years old.


2. I state that throughout the marital life, I have always showed utmost

love, affection and treated Ms. __________with respect and have


always adjusted and compromised with her in every situation.

3. I state that due to various marital discords, I have been constrained to

stay separately since __________.

4. I state that since __________, I have made several attempts for


reconciliation or amicable resolution of all the marital differences
between us and to initiate a peaceful life. However, all the efforts

have gone in vain as Ms. __________has failed to appreciate or


reciprocate the efforts made by me for a peaceful marital life or an
amicable resolution of all the issues between us.

5. I state that I am very attached to my daughter, whom I have not been


allowed to meet since August, 2020 on one pretext or another. After
several attempts made, finally, I was allowed to meet my daughter
once recently on 28th February, 2021 for a few minutes. All efforts of
mine to meet and build a meaningful relationship with my daughter
has been sabotaged. My daughter is also very attached to me. I am a
responsible father and have always taken all the responsibility of my
daughter. I am also willing to take sole custody and guardianship of
my daughter.

6. I state that various efforts have been made by me for reconciliation or

to amicably resolve our disputes. Although Ms. __________has


expressed her interest in mutually dissolving our marriage, however,
has failed to come forward to proceed with the same. I state that we
had also exchanged proposed terms and conditions for mutual consent
divorce through our lawyers, but the same are in vain. I am still
willing to make every effort towards reconciliation or amicably resolve
the disputes between us through talks and mediation process and
hence I am approaching your good offices to call upon Ms.

__________for mediation before initiating any legal proceedings


against her before the concerned Hon’ble Court, in the interest of
arriving at a mutual amicable settlement of our issues.
