You now have a completed Child Travel Consent Form. Complete the following steps to
put the document into effect and ensure it will be legally valid.
Ensure You and Your Child Know Why the Form is Required
You and your child need to understand when a Child Travel Consent Form is necessary.
This is true for any minor under 18 and especially true if your child is under the age of 16 as they
may be stopped and questioned by travel authorities when traveling alone.
The form is required when:
• A child’s parents are separated, and the child is only traveling with one parent.
• A minor is traveling without their parent(s) on a school trip.
• A child travels with a group, such as a sports team or a religious or cultural group.
• A child travels with a temporary guardian or any family member that is not a parent.
• A child travels alone via plane, train, etc.
The form should be provided regardless of whether the child is traveling domestically or
internationally.
However, if your child is traveling to another country, it will be especially critical for them to have
the form correctly filled out.
Once you know the exact reason the Child Travel Consent Form is necessary, you can move on to
the next step.
Receive Consent from the Other Parent if Necessary
Receive approval and the signature of the other parent if you are still together or share custody of
the child.
Some circumstances allow you to bypass the other parent's approval, those being:
• You have full custody of the child (by presenting a court custody document).
• You are the only surviving parent.
If the other parent is deceased, you will likely need to provide a death certificate to travel
authorities.

Review the Completed Child Travel Consent Form
Before signing the document, review your completed form to ensure that all information has been
added correctly.
The form must include the following:
• Child’s name, date of birth and location, and passport information.
• Child’s allergies and special needs.
• Authorization to travel alone or with another parent, individual, or group domestically, or
internationally.
• The name of the person the child will travel with if not alone, and that person’s contact and
passport information.
• Parent(s) information, and any necessary custodial details.
• Child’s travel destination and travel details such as date of departure and return.
After reexamining this essential information and confirming that is included and accurate, you can
move on to the final step.
Sign and Notarize the Document
After ensuring all the information is listed, it will need to be signed to put it into effect.
Both parents (when applicable) must sign and date the document.
It is also strongly suggested that you notarize the document. Numerous travel authorities require
the document to be notarized or the child may not be allowed to travel, whether domestically or
internationally.
It will almost always be necessary to notarize the consent form before an international trip. Many
countries have strict rules regarding the entry of unaccompanied minors and require that the form
be notarized.
Always check with the travel requirements of the destination beforehand to know if your form
must be notarized.
