Job content principle responsibilities
The post holder will work as a member of the Evening Nursing Service team, covering 7 evenings per
week, working from 6pm until 1130pm or 645pm - 1145pm covering 7 evenings per week.
The post holder needs to be a car driver and have a car available for the work, the expectation of the
post holder is that they will drive if working with a trained member of staff. Mileage will be paid.
Job Summary:
The post holder will work as a member of the Evening Nursing Service providing direct and indirect
patient care as directed by a trained member of the team in order to support patients and their families
within their own home environment. The post holder will undertake clerical duties as directed including
data inputting, and working as an effective member of the Multidisciplinary team.

JOB DESCRIPTION
Main Duties and Responsibilities
To provide direct and indirect patient care under the direction of a senior member of the team this will
include:
Helping staff to move and handle patients after training
Assisting senior staff in clinical procedures
Observations of patients on each visit under the direction of a senior member of the team.
Chaperone senior staff members when undertaking home visits
To undertake data inputting for the post holder and other team members as directed after
training.
Undertake clerical work as directed.
Maintaining accurate patient records in line with the Trusts record keeping policy.
Comply with the Trusts clinical standards.
Identify personal training needs and be prepared to undertake relevant training in order to develop the
role and the team, this training may be during day time hours.
Maintain confidentiality at all times ensuring all information regarding patients and staff gained through
your employment with the Trust is confidential. Disclosure to any unauthorised person is a serious
disciplinary offence.
Comply with Trusts policies for personal and patient safety and for prevention of healthcare associated
infection (HCA1). This includes a requirement for rigorous and consistent compliance with Trust policies,
procedures and guidelines in relation to all aspects of infection control.

Maintains accurate records of all nursing interventions and other activities associated with work in line
with Trust policies and Professional guidelines.

Ensure effective use of material resources/supplies within the team in consultation with senior staff.
Ensure patients valuables and belongings are documented and managed according to Trust policy.
Monitor, control and store resources/supplies according to the requirements /specifications of the clinical
environment. Participate in ensuring appropriate usage and monitoring of departmental resources.
Responsibility for Policy and Service Development and Implementation

and staff, whether in paper, electronic or other media, in accordance with the Data Protection Act 2018
and the General Data Protection Regulations (GDPR), ensuring the security and confidentiality of data at
all times.
The post holder must not for their own benefit or gain, or to divulge to any persons, firm or other
organisation whatsoever, any confidential information belonging to the Trust or relating to the Trust’s
affairs or dealings which may come to their knowledge during employment.
Research, Development and Audit
Participates in audit and benchmarking within the team
Physical Skills and Effort
Responsible for maintaining nursing bag with equipment required to deliver care e.g. Dressings, syringes
and needles. Occasionally required to carry emergency loan equipment to patient’s homes e.g.
commode.
Regularly involved in moving of patients by hoisting, moving of patient on bed by rolling or use of slip
sheet, manually moving equipment.eg. Commode.
Drives between work locations.

Hand Hygiene
To assist the Trust in reducing healthcare acquired infections (HCAI’s) all staff should be familiar with all
the Trust’s Infection Prevention policies which are appropriate to their role. You are required to attend
mandatory training in Infection Prevention and be compliant with all measures known to be effective in
reducing HCAI’s”
Safeguarding
All Stockport Foundation Trust employees are required to act in such a way that at all times safeguards

management of long term conditions.

Health & Safety
Under the Health and Safety at Work Act 1974, the Trust has a statutory responsibility to provide and
maintain a healthy and safe environment for its staff to work in. Equally the post holder is required to
work within the Trust Health & Safety Policy and fulfil a proactive role towards the management and
control of risk. This entails the identification, assessment and the immediate reporting, using the Trust
Incident Reporting System, any incident, accident, hazard or near miss involving patients, service users,
carers, staff, contractors or members of the public.

No Smoking Policy

comply with this policy, failure to do so may result in disciplinary action being taken under the Trust’s
