1.0 INTRODUCTION

We proposed implementing an appointment management system to handle the patient’s


information details, type of treatments, date and time in this proposal paper. My team members
and I will create this system for D’JIA Dental Clinic.The team members are Anis, Izzah and
Jannah will help to create this system successfully. For this company, we have examined the
requirements that must be fulfilled in the new system. The company that will implement this
system is D’JIA Dental Clinic, which offers the services of treatment to the people in the whole
country, which is Malaysia. This company was founded in early April 2012. This company gives
the best treatments just focusing on dental only. There are many treatments in this clinic such as
scaling, filling, tooth extraction, braces, Root Canal Treatment(RCT), Minor Oral Surgery(MOS)
and others. The clinic has 3 dentists who have many experiences in the field of dentistry.
Furthermore, D’JIA Dental Clinic does not have a system yet to book an appointment. As for
now, if a patient wants to book an appointment, they must contact the clinic or whatsapp and
they will come on that day that they have booked.It will cause such problems to the clinic if the
patient does not follow the instructions for making an appointment.

We summarized and highlighted the issues faced by the organization in order to handle a
list of booked appointments in the current problem statement section. We describe the project’s
objectives and include the Entity Relational Diagram (ERD) in Crow’s Foot notation, which
describe the project’s main concept. In the flow of the application part,we also explain the plan
CRUD (create,read,update, and delete) activities.

3.0 OBJECTIVE

I. Digitalize the data

● In order to make the appointment booking easier, we need to digitize the data
where patients can make an appointment through a website and they also can
select the date and time that they want to book but by looking at the emptiness
that exists at that time. By making an appointment via website, the patients also
will get a confirmation email that can validate the patients booking appointment
when they come to the clinic.

II. Improve data performance

● File based system actually stores data in the form of isolated files which have
their own set of property tables and physical location on the drive and the user
manually goes to these locations to access the files. In order to prevent the
overlapping or redundant data of patient’s appointment, the stored data of patients
appointment in the book needs to change to the digitized data, where all patients
appointments can be seen and updated via the website by the clinic staff. It also
will make the appointment journey easier as the patients also will get the valid
appointment date and time without worrying about the other patients as everyone
has a different time of appointment.

III. Reduce no-shows

● When patients miss appointments, it bothers the clinic as it costs the clinic money
and time. For every patient that does not make it to their appointment, the clinic
will lose out on revenue. An online appointment scheduling system can reduce the
no-shows dramatically by sending out reminders. The appointment scheduler
connects with clinic patient’s online calendars. Their calendar will remind them
about their appointment while giving them an appropriate amount of notice. The
dental clinic can also schedule SMS reminders through the system.

4.0 FLOW OF APPLICATION

4.1 Flowchart

This is the flowchart for our website, where it will show the flow that the patients need to
undergo to make a booking appointment with D’JIA Dental Clinic.
