RE: ADEYINKA SHERIFAT TEMITOPE / FEMALE / 27 YEARS
The above named presented at the medical out patient department of the hospital is
suffering from a severe ulcer pain. Her last exacerbation warranted her being admitted
for urgent medical treatment. Now discharged and placed on medications which she is
fully yet to respond to.
She Is also been monitored closely to ensure compliance and adequacy of dosing.
However, patient has been advised to avoid all strenuous activities as well as placed on
special diet and will be appreciated if she is given preferential attention on basis of any
request pertaining leverage to stay close to an area where she can easily get to the
hospital for proper management and care by her personal physician and hospital at
large.
We do solicit for a redeployment to her state where she has been receiving treatment
(OYO STATE).We anticipate your thoughtful consideration and favourable disposition.
