Sub: Subscriber Verification Process for Activation of a New Mobile Connection
The Cellular Operators Association of India (COAI) and the Association of Unified Telecom Service
Providers of India (AUSPI) together represent the mobile operators in India.
In order to comply with the subscriber verification guidelines of the DoT and also address the
concerns of national security agencies, COAI and AUSPI have come together on a common platform
and set up the Apex Advisory Council for Telecom in India (ACT) to oversee and ensure that the
proper subscriber verification process is followed before any
It is thus brought to your notice that in order to comply with the Government of India
Guidelines on Subscriber Verification, it is your duty as the Point of
1, NO PRE-ACTIVATED SIM CARDS ARE SOLD by you. Pre-activated SIM cards are those
which are in such a condition that it is possible to make and/or receive calls/SMS without
following the activation procedure laid down in the DoT guidelines. It may be noted that
activation will be done only by the employee of the Licensee. At the first stage of SIM activation,
subscriber will only be allowed to make a call to the operator for being tele-verified. Full
services will be activated, i.e. make and/or receive voice and/or nonvoice calls, only after the
subscriber has been tele verified.
2. When an Applicant approaches you for a mobile connection, you shall ensure that:
a. Four passport size photograph, one photograph is pasted in the space provided on the
Customer Application Form (CAF) and signed across by the applicant, overlapping on both the
photograph and the CAF.
b. Applicant submits self-attested (signature) clear and legible photocopy of Proof of Identity
(POI) and Proof of Address (POA) and is attached with the CAF. List of document acceptable
as POI and POA are given in Annexure-1.
c.The self-attested copies of POI & POA are matching with their respective originals.
d. The CAF is duly filled and signed by the applicant with date.

e. All Mandatory fields are filled in the CAF.
f. In case of illiterate/physically disabled applicant, the mandatory fields in the CAF may be filled
by POS but the thumb impression of the applicant is to be taken on CAF,photograph and the
supporting documents (POI & POA).
g. The applicant is to be provided with the counterfoil receipt, having details duly filled in and
signed with stamp of POS.
h. POS shall record in the CAF that he has
i. Seen the subscriber
ii. Matched the photograph attached to the CAF with the subscriber present
iii. Verified copies of POI and POA with the original
iv. Put his signature along with full name and stamp containing address on the CAF
and all attached documents
3. In case of outstation subscriber:
An outstation subscriber is one who does not have Proof of local address of the same licensed
telecom service area or the State/Union Territory concerned, whichever is more encompassing.
The POS shall additionally obtain the name, address and contact phone number of local
reference on the CAF. POS should telephonically verify the local reference before issue of SIM
Card and also make an entry in the CAF along with the name and number of local reference
contacted.
4. In case of Foreigner/ short time mobile connection:
OS shall obtain copy of passport with valid visa stamp as POI. For POA, the address of local
reference, which may also be of tour operator, shall be taken. In case of no local reference, the
address of stay (hotel, etc.) is to be indicated.
Subscriber shall be informed that any mobile connection issued to a foreigner shall not have
validity beyond the validity of the Visa. In case of foreign tourists, the validity shall not be
beyond Visa period and shall also not exceed 3 months at a time even if the validity of the Visa
is beyond 3 months.
In case of mobile connections issued to personnel deployed on ships while at seashore, the
connection shall be valid only till the date they have the permit to stay at the port.
5. In case of Bulk Connections:
Bulk mobile connection shall mean 10 or more than 10 connections issued in a single name at
any given address by all licensed operators in the service area. This would include connection
from the same operator or other operators.
Subscriber shall be informed that Bulk connections shall not be provided to individuals.
Subscriber shall declare in the CAF number of mobile connections held in his name with other
operators (operator wise).

6. In case of change of Name :
Change in name of subscriber is not permitted as the SIM card in the User terminal is non-
transferable.
Change in the name between blood relatives /legal heirs is permitted provided new CAF and all
the procedure for registering a new subscriber is followed and new SIM card is issued. In case
of change in name, change of address is not permitted. Further, No Objection Certificate (NOC)
from the Original User shall also be taken. In case of death of original user, death certificate will
suffice instead of NOC.
7. In case of change of address:
Subscriber shall be informed that service provider is to be intimated in case of change of
address within one week of such change along with new POA. If it is found that subscriber
address is not correct in the database, the connection shall be disconnected.
8. In case of forged documents:
In case you find that the subscriber submits a forged document and/or originals are also forged,
do bring it to the notice of the Licensee immediately and file a police complaint against such
subscriber within 15 days of bringing it to the notice of the Licensee.
9.Please Note that:
a. Details in CAF must match with that in POI, POA or any other supporting
b. CAF has been duly filled and signed by Applicant in your presence.
c. You should be able to establish that POI and Photograph is of the person present.
d. You have seen the subscriber and verified his documents with the original.
In case of illiterate/handicapped applicant, thumb impression of the applicant is to be taken.
a. The subscriber shall be provided a counterfoil/ receipt of the details of the PoI & PoA clearly
mentioning the name of subscriber, Mobile number applied for, CAF number, type of PoI/ PoA,
issuing authority, date of issue and serial number of PoI/ PoA document duly signed with stamp
of PoS.
If the documents are COMPLETE and ACCEPTABLE as detailed above, then you certify the
same your signature, name, date and stamp in the specified space/column provided in the CAF
and on all supporting documents and issue the SIM Card to Applicant and forward the
documents to the Dealer/Authorized
If documents are INCOMPLETE or there are any deviations vis-à-vis the requirements laid
down above,No SIM card is to be issued to the Applicant.
It is mandatory to follow each of the above steps before sending the CAF and supporting documents
to the Dealer/Distributor/Company, who then process the same for Activation of the
connection.
The same procedure must be followed for both postpaid and prepaid mobile

You have a duty to ensure that the above guidelines are strictly followed before issuing the
SIM Card to new customer.
Failure to comply will result in appropriate legal action.
Please note that no telecom service provider employee is authorized under any
circumstances to induce, encourage, and coerce any distributor or retailer to violate any of
the policies and procedures enunciated above. If such activity is observed by you
(distributor or retailer) you are duty bound to inform (in writing or by an e-mail) this to the
Authorized Officer of that employee’s telecom service provider company, within 24 hours of
