incurred due to an injury I sustained on 3rd December 2024, while on duty in the school. My
left leg was badly fractured in the incident, and I am currently undergoing surgery and
treatment as advised by the medical professionals, in CMH Lahore.
Given the severity of the injury and the subsequent medical procedures, I am facing a
significant financial burden due to hospitalization, surgery, and the ongoing recovery process. I
kindly request your support in the form of relief to help alleviate these unexpected medical
expenses.
I have attached the necessary medical documents, including the doctor's certificate, medical
bills of amount----- so far expended, for your reference and further processing of my request.

Subject: Application for Leave due to Leg Fracture
Respected Principal,
I am writing to inform you that I have suffered a leg fracture and will be unable to
attend school for the next 30 days from 4th December to 2nd January,2025.
I kindly request your permission to take leave from school during this period. I will
provide a medical certificate from my doctor to confirm my injury and the
expected recovery time.

on any missed schoolwork as soon as possible.
