To Whom It May Concern:
Please be informed that the College of _______________________________________

son/daughter /ward is a student will undertake an educational activity.
Activity: UC INTRAMURALS 2024 - GAMES AND TRAININGS

While we encourage all the students to participate in this learning activity, we require them to
present a signed written consent (through the WAIVER AND RELEASE portion) regardless whether he/she will
be allowed to join by the concerned parent/guardian.
WHEREAS, this university allows this activity only upon the free volition of the students with the written
consent of their parents/guardian, and under the supervision of their Adviser/s who shall faithfully and
honestly observe due diligence and care within the bounds of the law, to prevent any untoward incident/s,
to uphold the rules and regulations of the university governing such activity and the violation thereof will
make the person/s concerned liable;
WHEREAS, this activity shall also be governed by the following rules:
A. For Adviser/s:
1. No undue pressure, either directly or indirectly, shall be exerted on any student in order to
participate;
2. Advisers shall diligently guide the students in accordance with rules and regulations
governing school activities and the conduct of students outside the school;
3. The responsibilities of the advisers are limited only at the very start of the activity until its
completion;
4. The advisers are responsible for the consequences of their negligence;
5. The advisers are not liable for any untoward incident due to:
i. Violation of the student/s on the rules and regulations or instructions of the adviser;
ii. Negligence on the part of the student/s;
iii. Unforeseen events by nature;
iv. In their absence, which is not part of the period covered;
v. Force majeure, including disturbances or riots; and
vi. Assumption of risk by the student/s themselves;
B. For Student/s:
1. Student activities are governed by school rules and regulations;
2. Students are required to observe and follow the rules and regulations of the University as
stated in the Student Handbook;
3. Students are required to observe and follow the regulations and instructions impose by the
advisor/s before and or during the activity as warranted;
4. Students are responsible for any act or untoward incident if the cause is:
i. Due to their own negligence;
ii. Due to their deliberate violation of the rules and regulations;
iii. Not covered by the period defined under the adviser’s duties and responsibilities; and
5. Students can participate in the activity only if they have obtained Parental/Guardian’s
CONSENT and on a voluntary basis.
6. Along with the obtained parental/guardian’s CONSENT, student participant is also required
to submit a valid photocopy of the signatory’s identification card which any of the following:
company id, voter’s id, postal id, passport copy, driver’s license, professional id issued by PRC,
SSS, GSIS or any government and private institution issued id.

permission to go on field or assignments outside or around Baguio City. We are aware of the risks
that our son/daughter may be exposed to during these learning activities. Just the same, we allow
him/her without any reservation to participate in these learning activities for the duration of his/her
course. While he is participating in the said learning activities, we hereby voluntarily and expressly
waive any or all actions, claims, or demands against University of the Cordilleras, its trustees,
officers, teachers and/or employees should our son/daughter suffer any injury or damages which
may happened beyond the control of the faculty; and we hold harmless the said University of the
Cordilleras, its trustees, officers, teachers, and/or employees from any such action, claim or
demand.

utmost care, attention and precautions are undertaken and/or exercised by those concerned.
