WIAB Feedback form

Thank you very much for completing the Workshop in a Box. This document sets
out the questions you will be asked to answer on the online portal when feeding
back insights. Go to the community engagement page to complete the feedback
form online.

When completing the form, remember:

• To provide summaries that capture the balance of the conversation that


you had. You do not need to share verbatim feedback, although if you
would like to include one or two illustrative quotes you can do so
• Not to include any personally identifiable information about the people you
spoke to
• If you did not cover all of the questions in the workshop, that’s absolutely
fine – just skip the sections you did not include in your session
• If you have delivered more than one Workshop in a Box, you should
submit a separate form for each workshop
