A-Judicial-Affidavit Bjohn
I, Al-fauzi Ibno, 28 years old, born on February 12, 1996, single, employed with
the Philippine National Police stationed at Zamboanga City Forensic Unit detailed as
Polygraph Examiner, after having been duly sworn to in accordance with law, hereby depose
and state:
PRELIMINARY STATEMENT
That in accordance with A.M. No. 12-8-8-SC, which prescribes the use of judicial
affidavits to serve as the direct examination testimony of the witness, on the basis of which
the adverse party may conduct their cross-examination on such a witness, I hereby execute
this judicial affidavit in a question-and-answer format;
That conformably with section 3 (b) of the said A.M. No. 12-8-8-SC, I also state
that it was PSMS Maejor G. Coronel, investigator of the case who conducted the examination
of the undersigned affiant;
That conformably also with section 3 (c) thereof, I hereby state under the pain of
perjury that in answering the questions asked of me, as appearing herein below, I am fully
conscious that I did so under oath, and that I may face criminal liabilities for false testimony
or perjury;
1. Q : Please state your name and other personal circumstances for the record.
A : I am Al-fauzi J. Ibno, of legal age, single, and a Polygraph Examiner for the PNP
Forensic Unit, Zamboanga City.
2. Q : Mr. Witness, you stated that you are a Polygraph Examiner, will you state some of
your qualifications, if any?
A : I have undergone relevant training and have acquired the necessary qualifications
to conduct polygraph examinations.
3. Q : Aside from said qualifications, have you passed/received Civil Service Eligibilities,
trainings, and work experiences, received certifications, appointments and did research
papers?
A : I have successfully completed accredited courses in polygraph examination
techniques, and I am familiar with the relevant scientific principles and methodologies.
Equipment Setup: Conducting set-up and check the equipment to make sure it works right
during the test.
Conducting the Examination: During the test, the examiner observes physiological
responses using sensors, then asks questions while keeping an eye on how the body reacts
through the presentation of graphs.
Analysis and Interpretation: After the test, the examiner reviews the data collected and
establishes if the person was deceptive or not.
Reporting and Documentation: Then, they write a detailed report with the test details and
their conclusions. This report goes to the person who asked for the test.
Testimony and Expert Witness: Sometimes, examiners may testify in court about the test
and the result. They explain the process and share their expert opinion.
7. Q : Have you ever examined a man by the name of Andres Camuna y Mejia?
A : Yes
10. Q : I have here a document from PNP Zamboanga City Crime Laboratory Office
entitled Polygraph Examination Report dated April 23, 2024, is this familiar?
A : Yes.
11. Q : At the bottom of this document entitled Polygraph Examination Result, there
appears to be a name, Al-fauzi J. Ibno with a signature over it. Is this your name and
signature?
A : Yes.
