To Be Signed with Notary – Original Document Must Be Received


You must present a valid government-issued photo ID and complete this form in front of a licensed notary public. The ID that
you present to the notary must be the same ID that you photocopy and submit to SNHU. Note that alterations (i.e. crossed out
information or use of Whiteout) cannot be accepted. This form must be filled out in its entirety; missing information will not be
accepted.
