Lessons learned as a practice cover the procedures required for lesson identification, archiving,
validating, and disseminating. Utilizing and incorporating those processes entails identifying
applicable lessons learned, documenting lessons learned, archiving lessons learned,
distributing lessons learned to the appropriate personnel, identifying the actions that will be
taken as a result of the lesson learned, and monitoring the situation to make sure the right
actions were taken.
What Is a Lessons Learned Document?
Lessons learned are the information that has been recorded and reflects both the successful
and unsuccessful aspects of a project. They stand for the organization's dedication to
excellent project management and the project manager's chance to gain knowledge from others'
real-world experiences.
How to Document Lessons Learned?
Lessons learnt should be recorded and then communicated to project stakeholders.
Depending on the audience, many sorts of reports can be created. The information
gathered at the lessons learned session and any extra comments from participants who
could not attend are included in the detailed lessons learned report. All participants
should receive a copy of the thorough lessons learned report from the facilitator and
have the opportunity to comment on its accuracy. Even if they did not participate in the
lessons learned session, the whole project team should receive a copy of the report
once it is finished. The final report needs to be kept with the other project records.

respect to learning lessons to be undertaken, and good practice guidelines on how to learn lessons and
share information to be drawn out. For effective system for learning lessons from incidents would need
to include the following elements:
. An incident/accident reporting system;
. A process for incident investigation that ensures that the underlying as well as immediate causes of
accidents and incidents are understood, taking full account of human and organisational factors;
. A process for analysing cumulative information on accidents and incidents from both internal and
external events;
. A process for ensuring that the findings of incident investigation and analysis of accident and incident
data are acted upon in a timely fashion and suitable interventions put in place or modifications made to
prevent a recurrence of the incident or similar incidents;
. A process for evaluating the success or otherwise of interventions and modifications;
. A process for disseminating information on accident and incident causation and suitable
interventions/modifications to all relevant parties (both internal and external), as quickly as possible;
and
. A system to capture the information in a format that is readily searchable and retrievable to allow ease
of access, so that any lessons learned stay learned (corporate memory).
