DECLARATION BY THE STUDENT & HIS/HER FATHER/ GUARDIAN

1. I do solemnly affirm that the statement and information furnished by me overleaf are true. I
know that, if any information furnished therein is found to be untrue in material particulars, I
shall be liable to criminal prosecution as well as to forgo my admission fee and to be taken out
of the Institution, if already admitted.
2. I also undertake to pay the Institution, Hostel and other dues regularly and in time.
3. I promise to abide by all the rules & regulations of the Institution as well as those of the State
Govt./ Education Department / Institution concerning fee structure, Examination, attendance,
practical training etc.
4. I also understand that the following will be deemed to be the acts of indiscipline:
a. Non – compliance of the Institution and Hostel rules, orders and notices.
b. Noncompliance of orders and instruction of Institution, Management and Faculty member.
c. Involvement in RAGGING:- (As per the Directive from Govt. of India) Ragging in any form
by students will result in immediate disciplinary action. Students found guilty may be
summarily expelled from the Institution/Hostel.
d. Irregularity in attendance, in payment of Institution dues and in any other related matters.
e. Misuse of Institution properties.
f. Provoking students to take part in strikes, morchas or formation of Union/Associations the
objective of which is directly or indirectly political or communal.
g. Arranging meetings or inviting some person/persons to address any meeting in the
Institution, Hostel premises without prior permission of the Institution.
h. Smoking, chewing, Tabaccoo & Gutka use of any drugs, alcohol in the Institution / Hostel
premises is prohibited.
i. Involvement in any offence / crime punishable under Institution rules or a Court of Law.
5. I also understand that if I am found involved in act of Indiscipline, I am liable to any one
or more of the following punishment:
a. Written warning to me and my Father/Guardian by the Institution Management.
b. Recovery in part or full of losses or damages to the Institution property or property of
others along with fine.
c. Suspension from availing of any of the Institution amenities and services or from classes.
d. Removal / Rustication or Expulsion from the Institution.
6. I also declare that:
a. I have not been convicted of any criminal offences or have been released on bail in
connection with a criminal case.
b. No case of criminal offences or moral turpitude is pending against me in any court of law.
c. No complaint or FIR has been lodged against me by the Institutions or any other competent
authority.
d. I have not resorted to any act or indiscipline during the previous years.
