ICOPE definition

ICOPE, which stands for Integrated Care for Older People, is a framework
developed by the World Health Organization (WHO) to support the health and
well-being of older adults. It focuses on preventing, slowing, or reversing
declines in the physical and mental capacities of older people through
integrated care.

It is very difficult to measure intrinsic capacity of an older individual, but the


multiple domains that contribute to this totality can be assessed. The 5-key
domain of intrinsic capacity are:

 Locomotor capacity
 Vitality
 Sensory capacity
 Psychological capacity
 Cognitive capacity

ICOPE Interpretation

The ICOPE assessment helps identify declines in an older person’s intrinsic


capacity across five domains. The interpretation of the results can guide the
development of personalized care plans:

 Robust: High scores across all domains indicate a high level of intrinsic
capacity and independence.
 At Risk: Moderate scores suggest some decline, signaling the need for
preventive interventions.
 Frail: Low scores across multiple domains indicate significant decline
and the need for comprehensive care and support.

Barthel index definition

The Barthel Index is an scale used to measure a person’s ability to


perform ten basic Activities of Daily Living (ADLs). It was originally
published in 1965 and is widely used in clinical settings to assess the
functional independence of patients, particularly those with physical
disabilities due to neurological conditions.

Key Points of the Barthel Index:

 Assesses ADLs: It evaluates essential self-care tasks such as


feeding, bathing, grooming, dressing, bowel and bladder control,
toilet use, transfers (e.g., from chair to bed), mobility on level
surfaces, and stair negotiation.
 Scoring System: Each activity is scored based on the individual’s
ability to perform the task independently, with a total possible score
ranging from 0 to 100. Higher scores indicate greater
independence.
 Use in Rehabilitation: The index helps to determine the level of care
a patient requires and to monitor progress over time, especially
after rehabilitation interventions.
 Versions: There are several versions of the Barthel Index, including
the original and modified versions, which vary slightly in their
scoring methods.

The Barthel Index is simple to use and does not require formal training for
administration, making it a practical tool for various healthcare
professionals to assess a patient’s functional status and independence.

Instructions: Choose the scoring point for the statement that most closely
corresponds to the patient’s current level of ability for each of the
following 10 items.Record actual, not potentiai, functioning. Information
can be obtained from the patient’s self-report, from a separate party who
is familiar with the patient’s abilities (such as a relative), or from
observation. Refer to the Guidelines section on the following page for
detailed information on scoring and interpretation

FEEDING

0 = unable

5 = needs help cutting, spreading butter, etc., or requires modified diet

10 = independent

BATHING

0 = dependent

5 = independent (or in shower)

GROOMING

0 = needs to help with personal care

5 = independent face/hair/teeth/shaving (implements provided)

DRESSING
0 = dependent

5 = needs help but can do about half unaided

10 = independent (including buttons, zips, laces, etc.)

BOWELS

0 = incontinent (or needs to be given enemas)

5 = occasional accident

10 = continent

BLADDER

0 = incontinent, or catheterized and unable to manage alone

5 = occasional accident

10 = continent

TOILET USE

0 = dependent

5 = needs some help, but can do something alone

10 = independent (on and off, dressing, wiping)

TRANSFERS (BED TO CHAIR AND BACK)

0 = unable, no sitting balance

5 = major help (one or two people, physical), can sit

10 = minor help (verbal or physical)

15 = independent

MOBILITY (ON LEVEL SURFACES)

0 = immobile or < 50 yards

5 = wheelchair independent, including corners, > 50 yards

10 = walks with help of one person (verbal or physical) > 50 yards

15 = independent (but may use any aid; for example, stick) > 50 yards

STAIRS

0 = unable
5 = needs help (verbal, physical, carrying aid)

10 = independent

Score interpretation:

BADL total score Interpretation

 Moderate disability <15


 Severe disability <10

Lower the score; higher is the disability which is proportional to the


amount of care Demanded by the patient.

Lower the premorbid score; poorer are the outcomes of the patient.

Barthel Index Interpretation

The Barthel Index measures performance in 10 ADLs with a score range


from 0 to 100:

 0-20: Severe dependency.


 21-60: Moderate dependency.
 61-90: Mild dependency.
 91-99: Low dependency, but not completely independent.
 100: Fully independent in physical ADLs.

IADL assessment tool definition

The Lawton Instrumental Activities of Daily Living Scale (IADL) is an


appropriate instrument to assess independent living skills (Lawton &
Brody, 1969). These skills are considered more complex than the basic
activities of daily living as measured by the Katz Index of ADLs (See Try
this: Katz Index of A”Ls).

The Lawton IADL scale can be scored in several ways. The most common
method is to rate each item either dichotomously (0 = less able, 1 = more
able) or trichotomously (1 = unable, 2 = needs assistance, 3 =
independent) and sum the eight responses. The higher the score, the
greater the person’s abilities.
