How to calculate the Maximum Dose of each type of local anaesthesia

according to body weight with vasoconstrictor?

 First step you have to know the Maximum recommended dose of


each local anaesthesia type mentioned in the following table
• Maximum recommended dose is a dose given by the
manufacturers after exceeding these doses potential local
anaestheisa toxicity may occur.

 Second step : you have to determine the pt. weight here we will say
that the mean Yemeni pt weight is 70kg
 So to calculate the Maximum dose we will use the following
equation
Pt. weight X MRD of the local anaesthesia for example lidocaine
which is the most commonly used local anesthetic
70 kg X 7 mg = 499 mg

 Third step : how to calculate the total amount in milligrams of LA


in the carpule use the following equation given that the size of LA
carpule in yemen is 1.8ml

Total milligrams= % of the solution X 10 X total millilitres


2% lidocaine X 10 X 1.8 which means
20 X 1.8 =36 mg lidocaine in each carpule

 Fourth step: how to calculate the maximum number of carpule of


lidocaine

Pt weight 499 = 13.8 carpule

How to calculate the amount of adrenaline in each carpule?

Use the following equation:


 1:80,000 = 1000 = 0.0125mg/ml

 1:100,000 = 1000 = 0.01mg/ml

 1:200,000 = 1000 = 0.005mg/ml

So the dose 1:80,000 adrenaline in 1.8ml carpule =
0.0125mg/ml X 1.8ml = 0.0225mg

How to calculate the maximum allowable vasoconstrictor of the cardiac


patient

The dose in pt. with cardiac disease should not exceed 0.04 mg
So if you have lidocaine 1:80,000 this means it contains 0.0225mg adrenaline
then you should not give him more than 2 carpules

How to calculate the dose of Local anesthetic agents without a


vasoconstrictor:
 The recommended dose of lidocaine, without a vasoconstrictor is 4.4
mg/kg BW, but not to exceed 300 mg.
 A lesser dose is recommended because of faster absorption of the local
anesthetic agent.
 This dose will be contained in 15 ml or 7½ carpule of the local anesthetic
solution.
