Subject: Issue of apology for inadequate attendance and request for reinstatement of my
name in the attendance sheet

Dear Sir,
I hope this letter finds you well. I am writing this letter to apologise on my behalf for the
unintentional absence of adequate attendance due to which my name has been removed
from the new attendance sheet.

As my father had been severely ill from lung infection, fever and chest disease and was
prescribed to take bed rest for 15-20 days from August 8(a copy of the prescription is
attached herewith), I was mentally exhausted and had to support and take care of him after
fulfilling the tasks he had been doing. Despite these adverse circumstances, I tried to attend
as many classes as was possible for me during the month of August. I also apologise for my
failure to inform my mentor and respective subject teachers about the situation as early as
possible.

Pursuing an undergraduate degree from Jadavpur University in Mathematics is very


important for me and I wish to continue it if given the chance. Without it, I would be
completely devastated losing an year of my studies. I humbly request you to reconsider the
situation empathetically and to consider reinstating my name in the attendance sheet.
