I am writing to formally submit my application for a Poland National D-Type Work Visa. I have
accepted a full-time position as a Cleaner with Linpez SP. Z O.O. located in Katowicka, 41-902
Bytom, Poland. My employment is set to begin on 8th April 2025 and continue through 30th
December, 2025. My position involves maintaining a clean and hygienic environment through
tasks like dusting, sweeping, mopping, vacuuming, restroom cleaning, and potentially minor
repairs, while adhering to safety and hygiene standards, and I am eager to contribute to the
company’s success while gaining international experience in Poland.

To support my application, I have enclosed the following documents:


 Completed and signed visa application form
 Valid passport
 Police criminal report
 Two recent passport-sized photos
 School Academic Certificates
 Birth Certificates and National Identification Card
 Official job offer letter from Linpez S.P Z.O.O
 Employment contract with Linpez S.P Z.O.O
 Proof of accommodation in Poland
 Valid travel insurance covering my stay in Poland
 Proof of financial means (bank statements, salary slips)
 Flight reservation details
 Work permit
