AFFIDAVIT TO SUPPORT A WORK PERMIT APPLICATION

This affidavit is in support of an application for a Work Permit under Section 66(10) of the Immigration (Transition) Law, 2018

1. That I am lawfully married/civil partner to

2. That my spouse/civil partner is a worker as defined in section 2 of the Law / a person employed by the Government of the Islands / a person employed by the Government of the United
Kingdom, in the Islands / a person employed by a statutory authority or Government owned company, the employees of which are by law not required to hold work permits / a person who
is working by operation of law under section 66(6) or with the permission granted under subsection (4) or (8) of the Law (Delete as necessary) and my right to work in the Cayman
Islands will expire before that of my spouse/civil partner;

3. That my marriage/civil partnership is not one convenience as defined in the Immigration (Transition) Law, 2018 and the Immigration (Transition)(Amendment)(No.2) Law, 2020;

4. That my marriage/civil partnership is stable and intact and I am not living apart from my spouse/civil partner under a decree of a competent court or a deed of separation;

5. That I had not lived apart from my spouse/civil partner for an aggregate period of three months out of the twelve months immediately preceding my work permit application.

Warning: It is an offence under the Immigration (Transition) Law, 2018 for any person to make, cause or allow to be made any return, statement or representation which is false in a material particular
and which he knows to be false or which he does not believe to be true. A person found guilty of this offence is liable on summary conviction in respect of a first offence, to a fine of $5,000.00 and to
imprisonment for one year or, in respect of a second or subsequent offence, to a fine of ten thousand dollars and to imprisonment for two years.

By making an application for the re-issue of a Residency & Employment Rights Certificate, you agree to cooperate with the Director of Work Opportunities Residency Cayman (WORC) /Caymanian Status
and Permanent Residency Board or its duly appointed agents by providing such information or documents as they may reasonably request in connection with your application.

If the Director of Work Opportunities Residency Cayman (WORC)/Caymanian Status and Permanent Residency Board has reasonable grounds to believe that any fact stated in any application for a
Residency and Employment Rights Certificate (including any affidavit sworn in such application) is false in a material particular, the Board or it's duly appointed agents may conduct a full investigation
in such manner as it deems fit.
